普伐他汀联合血必净注射液改善糖尿病肾病TGF-B、VEGF、HCY水平及其作用机制

摘　　要：目的分析普伐他汀联合血必净注射液改善糖尿病肾病患者转化生长因子-β(transforming growth factor-β,TGF-β)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、同型半胱氨酸(homocysteine,HCY)及其作用机制研究。方法选取医院在2019年12月—2021年3月收治的糖尿病肾病患者120例,采用随机数字法分为两组。他汀组采用马来酸依那普利片治疗,血必净组在他汀组基础上联合血必净注射液治疗。比较他汀组和血必净组Hcy、VEGF、TGF-β1、内皮素-1(ET-1)、凝血纤溶指标、糖代谢指标、肾功能指标。结果他汀组和血必净组治疗前Hcy、VEGF、TGF-β1、ET-1、凝血纤溶指标组间比较,差异无统计学意义(P>0.05)。血必净组治疗后Hcy、VEGF、TGF-β1、ET-1、脂蛋白[Lp(a)]、纤溶酶原激活物抑制物-1(PAI-1)低于他汀组,组织型纤溶酶原激活物(t-PA)、抗凝血酶-Ⅲ(AT-Ⅲ)高于他汀组(P<0.05)。他汀组和血必净组治疗前糖代谢、肾功能指标组间比较,差异无统计学意义(P>0.05)。血必净组治疗后空腹血糖(FPG)、餐后2 h血糖(2 h PG)、血肌酐(SCr)、β2-微球蛋白(β2-M)、尿白蛋白排泄率(UAER)、尿素氮(BUN)指标低于他汀组(P<0.05)。结论普伐他汀联合血必净注射液治疗糖尿病肾病,可调节相关血清因子的表达,改善糖代谢及肾功能,维持凝血纤溶系统平衡。Objective To analyze pravastatin combined with Xuebijing Injection(血必净注射液)to improve the levels of transforming growth factor-β(TGF-β),vascular endothelial growth factor(VEGF)and homocysteine(HCY)and its mechanism of action in patients with diabetic nephropathy.Methods A total of 120 cases of diabetic nephropathy from December 2019 to March 2021 were selected and divided into two groups by random number method.The statin group was treated with enalapril maleate tablets,and the Xuebijing group was treated with Xuebijing Injection on the basis of the statin group.The levels of Hcy,VEGF,TGF-β1,endothelin-1(ET-1),coagulation and fibrinolysis indexes,glucose metabolism indexes and renal function indexes were compared between statin group and Xuebijing group.Results The levels of Hcy,VEGF,TGF-β1 and ET-1 and coagulation and fibrinolysis in statin group and Xuebijing group before treatment had no significant difference between the two groups(P>0.05).The levels of Hcy,VEGF,TGF-β1,ET-1,lipoprotein[LP(a)]and plasminogen activator inhibitor-1(PAI-1)in the Xuebijing group after treatment were lower than those in the statin group,while tissue plasminogen activator(t-PA)and antithrombin-Ⅲ(AT-Ⅲ)were higher than those in the statin group(P<0.05).The levels of 2 h postprandial blood glucose(2 hPG),serum creatinine(SCR),β2-microglobulin(β2-M),urinary albumin excretion rate(UAER)and urea nitrogen(BUN)in the Xuebijing group were lower than those in statin group(P<0.05).Conclusion Pravastatin combined with Xuebijing Injection can regulate the expressions of related serum factors,improve glucose metabolism and renal function,and maintain the balance of coagulation and fibrinolysis system in the treatment of diabetic nephropathy.
