深圳市社区65岁及以上老年人糖尿病治疗和控制状况及其影响因素分析

摘　　要：目的了解深圳市社区老年人糖尿病治疗和控制情况及影响因素,为制定社区糖尿病综合防治策略提供依据。方法于2021年1—12月,采用方便抽样方法,抽取深圳市社区常住居民中351362名65岁及以上老年人作为调查对象,对其进行问卷调查、体格检查和实验室生化检测。采用R 4.1.2统计软件进行χ~2检验和多因素logistic回归分析。结果共检出83801名糖尿病患者,检出率为23.85%,糖尿病患者治疗率与控制率分别为64.49%和39.02%。多因素logistic回归分析结果显示,女性(OR=1.05,95%CI:1.01~1.09)、初中及以上学历(OR=1.14,95%CI:1.07~1.21)、丧偶(OR=1.15,95%CI:1.06~1.24)、已戒烟(OR=1.25,95%CI:1.19~1.32)、有体力活动(OR=1.19,95%CI:1.15~1.23)、中心性肥胖(OR=1.07,95%CI:1.03~1.11)、高血压(OR=1.06,95%CI:1.02~1.09)、血脂异常(OR=1.04,95%CI:1.01~1.07)和有心脑血管疾病史(OR=1.32,95%CI:1.27~1.38)的患者糖尿病治疗情况较好(P<0.01);年龄≥80岁组(OR=0.91,95%CI:0.86~0.95)、偶尔饮酒(OR=0.90,95%CI:0.86~0.95)、饮酒(OR=0.63,95%CI:0.59~0.68)、低体重(OR=0.79,95%CI:0.70~0.88)、超重(OR=0.80,95%CI:0.78~0.83)及肥胖(OR=0.69,95%CI:0.66~0.73)的患者糖尿病治疗情况较差(P<0.01)。年龄≥80岁组(OR=1.06,95%CI:1.01~1.11)、单身(OR=1.46,95%CI:1.02~2.09)、有体力活动(OR=1.05,95%CI:1.01~1.09)、低体重(OR=1.18,95%CI:1.06~1.32)、高血压(OR=1.33,95%CI:1.28~1.38)和有心脑血管疾病史(OR=1.41,95%CI:1.35~1.46)的患者糖尿病控制情况较好(P<0.05,P<0.01);70~74岁(OR=0.97,95%CI:0.93~0.99)、女性(OR=0.92,95%CI:0.89~0.95)、现在吸烟(OR=0.91,95%CI:0.86~0.96)、偶尔饮酒(OR=0.87,95%CI:0.82~0.91)、饮酒(OR=0.67,95%CI:0.62~0.72)、超重(OR=0.91,95%CI:0.88~0.94)、肥胖(OR=0.82,95%CI:0.78~0.87)、中心性肥胖(OR=0.79,95%CI:0.76~0.82)及血脂异常(OR=0.77,95%CI:0.75~0.80)的患者糖尿病控制情况较差(P<0.05,P<0.01)。结论深圳市社区老年人糖尿病检出率较高,控制率较低,应重点关注社区超重或肥Objective To understand the treatment and control and the influencing factors of diabetes among the elderly in Shenzhen community,and provide the basis for the development of comprehensive prevention and treatment strategies for diabetes in the community.Methods From January to December 2021,351362 elderly(≥65 years old)were selected from the permanent community residents in Shenzhen as the subjects by convenient sampling method.The investigation was performed with questionnaire,physical examination and laboratory biochemistry test.Theχ~2test and multivariate logistic regression analysis were used to analyze the data,the used software was R 4.1.2.Results A total of 83801 diabetic patients were detected,the detection rate was 23.85%,the treatment rate and control rate of diabetes were 64.49%and 39.02%,respectively.Multivariate logistic regression analysis showed that female subjects(OR=1.05,95%CI:1.01-1.09),and subjects with≥junior high school education level(OR=1.14,95%CI:1.07-1.21),widowed(OR=1.15,95%CI:1.06-1.24),quit smoking(OR=1.25,95%CI:1.19-1.32),physical activity(OR=1.19,95%CI:1.15-1.23),central obesity(OR=1.07,95%CI:1.03-1.11),hypertension(OR=1.06,95%CI:1.02-1.09),dyslipidemia(OR=1.04,95%CI:1.01-1.07),and history of cardiovascular and cerebrovascular diseases(OR=1.32,95%CI:1.27-1.38)had better treatment status for diabetes(P<0.01);the subjects with≥80 years old(OR=0.91,95%CI:0.86-0.95),occasional drinking(OR=0.90,95%CI:0.86-0.95),drinking(OR=0.63,95%CI:0.59-0.68),low weight(OR=0.79,95%CI:0.70-0.88),overweight(OR=0.80,95%CI:0.78-0.83),obesity(OR=0.69,95%CI:0.66-0.73)had poor treatment status of diabetes(P<0.01).The subjects with≥80 years old(OR=1.06,95%CI:1.01-1.11),single(OR=1.46,95%CI:1.02-2.09),physical activity(OR=1.05,95%CI:1.01-1.09),low body weight(OR=1.18,95%CI:1.06-1.32),hypertension(OR=1.33,95%CI:1.28-1.38),history of cardiovascular and cerebrovascular diseases(OR=1.41,95%CI:1.35-1.46)had better control status of diabetes(P<0.05 or P<0.01).The subjects with 70-74 years old(OR=0.97,95%CI:
