摘 要:目的 探讨2019—2021年浙江省水痘-带状疱疹病毒(varicella-zoster virus, VZV)分子流行病学特征,为水痘免疫策略与疫情防控提供实验室依据。方法 采用R软件分析水痘病例流行病学相关资料;采用实时荧光定量PCR方法筛选阳性样本;Illumina二代测序平台进行开放阅读框(open reading framme, ORF)ORF22和ORF54基因测序;应用生物信息学软件MEGA 11进行序列比对分析,采用单核苷酸多态性(single nucleotide polymorphism, SNPs)进行基因分型,同时与美国国立生物技术信息中心中已知的国内外水痘基因型株联配比对,构建系统进化树。结果 采集疑似水痘患者的疱疹液或咽拭子样本576份,选取Ct值较低、不同年份、不同来源的样本40份,2020年疱疹液阳性率高于2019年(98.15%vs 79.53%,P<0.01),发病高峰在11月。病例年龄主要集中5~<20岁(占93.06%),尤以10~<15岁青少年最多。SNPs和基因序列比对分析显示,样本感染的病毒均属于Clade2分支。结论 二代测序技术有助于快速准确检测VZV的基因型,Clade2是2019—2021年浙江省流行的VZV主要病原体,有必要对浙江省VZV流行株基因型别持续研究,并加强监测。Objective To explore the molecular epidemiological characteristics of varicella-zoster virus(VZV) in Zhejiang province, and to provide a laboratory basis for varicella immunization strategy and epidemic prevention and control. Methods R software was used to analyze the epidemiological data of varicella cases.A total of 576 samples collected from suspicious varicella cases in surveillance sites from 2019 to 2021 were tested by real-time PCR.ORF22 and ORF54 genes of VZV from 40 positive samples of herpes fluid or throat swabs with low ct value selected from different year and different sample type were sequenced on Illumina platform.MEGA 11 was used for sequence alignment and SNPs was applied for genotyping.Phylogenetic tree was constructed based on the known varicella genotypes downloaded from NCBI. Results The overall positive rate of herpes fluid varied in different years, which was significantly lower in 2019 than that in 2020(79.53% vs 98.15%, P<0.01).The number of cases peaked in November and majority were 5-<20 years old, accounting for 93.06%, especially adolescents aged 10-<15 years.Result of SNPs and gene sequence alignment revealed that all VZV isolates were Clade2 virus. Conclusions Next generation sequencing helps with the rapid determination of VZV genotypes.Clade2 VZV is the dominant genotype circulating in Zhejiang province from 2019 to 2021.The study findings will help to evaluate the necessity of varicella immunization in Zhejiang province.
