经阴道分娩实施会阴侧切分娩助产技术与自然裂伤缝合术对初产妇产后盆底功能的短期影响

摘　　要：目的研究会阴侧切分娩助产技术与自然裂伤缝合术对经阴道分娩初产妇产后盆底功能的短期影响。方法选取42例经阴道分娩初产妇,按照手术方法不同将患者分为对照组和试验组,每组21例。试验组实施会阴自然裂伤缝合术,对照组实施会阴侧切分娩助产技术。比较两组产后盆底功能及并发症发生情况。结果分娩后,试验组盆底收缩压、盆底静息压高于对照组,差异有统计学意义(P<0.05)。分娩后,试验组产妇的视觉模拟评分法(VAS)评分显著低于对照组,盆底肌肌电值高于对照组,差异有统计学意义(P<0.05)。分娩后,试验组并发症发生率为4.76%,低于对照组的28.57%,差异有统计学意义(P<0.05)。结论与自然裂伤缝合术相比,会阴侧切分娩助产技术虽然能够较好地防止产妇会阴裂伤,但对初产妇产后的盆底功能短期影响较大,并发症发生率较高。Objective To investigate the short-term effects of lateral perineal incision midwifery technique and natural laceration suture on postpartum pelvic floor function of primipara after vaginal delivery.Methods 42 cases of vaginal delivery primipara,according to different surgical methods were divided into control group and experimental group,each group of 21 cases.Natural perineal laceration suture was performed in the experimental group,while lateral perineal incision was performed in the control group.The postpartum pelvic floor function and complications were compared between the two groups.Results After delivery,the systolic pressure and resting pressure of the pelvic floor in the experimental group were higher than those in the control group,the difference was statistically significant(P<0.05).After delivery,the visual analog scale(VAS)score of the experimental group was significantly lower than that of the control group,and the electromyography value of pelvic floor muscle was higher than that of the control group,with statistical significance(P<0.05).After delivery,the complication rate of the experimental group was 4.76%,lower than 28.57%of the control group,the difference was statistically significant(P<0.05).Conclusion Compared with natural laceration suture,lateral perineal incision midwifery technique can prevent perineal laceration better,but it has a greater shortterm impact on postpartum pelvic floor function and a higher incidence of complications.
