左炔诺孕酮宫内缓释系统对宫腔镜电切术治疗的子宫内膜息肉患者子宫内膜厚度和复发率的影响

摘　　要：目的 探讨左炔诺孕酮宫内缓释系统对宫腔镜电切术治疗的子宫内膜息肉患者子宫内膜厚度和复发率的影响。方法 选取136例行宫腔镜电切术治疗的子宫内膜息肉患者,根据随机数字表法分为对照组与研究组,每组68例。两组患者均行宫腔镜电切术治疗,在此基础上,研究组患者采取左炔诺孕酮宫内缓释系统治疗,对照组患者不予以其他特殊治疗。比较两组患者临床疗效、子宫内膜厚度、并发症发生情况及复发率。结果 研究组治疗总有效率97.06%明显高于对照组的79.41%,差异具有统计学意义(P<0.05)。治疗前,两组子宫内膜厚度比较,差异无统计学意义(P>0.05);治疗后,两组子宫内膜厚度薄于治疗前,且研究组明显薄于对照组,差异具有统计学意义(P<0.05)。两组患者术后6个月内并发症发生率比较,差异无统计学意义(P>0.05);研究组患者复发率明显低于对照组,差异有统计学意义(P<0.05)。结论 对宫腔镜电切术治疗的子宫内膜息肉患者,术后宫内放置左炔诺孕酮宫内缓释系统可提高治疗效果,降低子宫内膜厚度,具有较高安全性,且有利于降低复发率,值得临床借鉴。Objective To investigate the effect of levonorgestrel intrauterine sustained release system on endometrial thickness and recurrence rate in patients with endometrial polyps treated by hysteroscopic electrotomy.Methods 136 patients with endometrial polyps who underwent hysteroscopic electrotomy were selected and divided into control group and study group according to random number table method,with 68 cases in each group.Patients in both groups were treated with hysteroscopic electrotomy.On this basis,patients in the study group were treated with levonorgestrel intrauterine sustained release system,while patients in the control group were not given other special treatments.The clinical efficacy,endometrial thickness,complications and recurrence rate were compared between the two groups.Results The total effective rate of the study group was 97.06%,which was significantly higher than that of the control group(79.41%),and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in endometrial thickness between the two groups(P>0.05).After treatment,the endometrial thickness of the two groups was thinner than before treatment,and the study group was significantly thinner than the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications within 6 months after operation between 2 groups(P>0.05).The recurrence rate of the study group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with endometrial polyps treated by hysteroscopic electrotomy,placing levonorgestrel intrauterine sustained release system after surgery can improve the therapeutic effect,reduce the endometrial thickness,have high safety,and is beneficial to reduce the recurrence rate,which is worthy of clinical reference.
