胎儿分裂手/足畸形产前超声特征:3例报告及文献复习

摘　　要：目的结合文献及3例临床所见,观察胎儿分裂手/足畸形(SHFM)产前超声特征。方法回顾性分析3胎SHFM胎儿,结合文献复习观察其产前超声特征。结果产前超声显示1胎(序号1)右手第3指骨缺失,左手仅3根手指,示指与小指间呈“V”形,环指与小指似为并指,右足自足底前1/3部分裂为两部分,呈“V”形裂隙,第2~4趾及其相应跖骨缺如,左足足掌中上部分裂为两部分,踇趾与第2趾似为并趾,第3与第4趾似为并趾;1胎(序号2)右手掌呈“V”形裂开,双足呈分叉状,部分足趾缺如;1胎(序号3)双侧手掌呈“V”形裂开,双足呈分叉状,部分足趾缺如。因孕妇要求终止妊娠而予以引产,3胎胎儿引产标本所见与产前超声表现一致。结论胎儿SHFM产前超声表现为双手/足呈“V”形裂开,伴指/趾骨及相应掌/跖骨缺如。Objective To observe prenatal ultrasonic characteristics of fetal split hand/split foot malformation(SHFM)combining with literature review.Methods Data of 3 fetuses with SHFM were retrospectively analyzed,and the prenatal ultrasonic characteristics were observed combining with literature review.Results In No.1 fetus,prenatal ultrasound showed that the third phalanx of right hand was absent,there were only 3 fingers in the left hand,the index finger and little finger were in"V"shape,the ring finger and little finger seemed to be joined,the right foot split into 2 parts from the front 1/3 of the sole of the foot with"V"shaped,the second to the fourth digits and the corresponding metatarsal bones were absent,the middle and upper part of the left foot split into 2 parts,the great toe and second digits seemed to be joined,and the third and fourth digits seemed to be joined.In No.2 fetus,the right palm of the fetus was split in"V"shape,the feet were bifurcated,and some toes were absent.In No.3 fetus,both palms of the fetus were split in"V"shape,both feet were like"bifurcation"shape,and some toes were absent.Then termination of pregnancy was performed in all 3 cases since requestion of pregnant women,and the findings of labor induction were consistent with those of prenatal ultrasound.Conclusion Prenatal ultrasonic characteristics of fetal SHFM included palms/soles split in"V"shape with absence of some finger/toe bones and corresponding metacarpal/metatarsal bones.
