中西医结合治疗干眼症研究进展

摘　　要：西医治疗干眼症多采用药物治疗、手术治疗及睑板腺治疗等,中医治疗则给予辨证论治中药内服、中药熏蒸、针刺治疗、针药结合治疗及超声雾化等,均可不同程度地缓解患者自觉症状,增加泪液分泌、延长泪膜破裂时间等。但仍存在以下问题:(1)中医治疗干眼症尚缺乏规范的诊疗标准,如疗程长短、药物使用剂量等;(2)西医治疗不良反应较多,且许多患者对药物有一定的依赖性;(3)中西医结合分类治疗方案的制定欠合理;(4)临床缺乏随访时间长、多中心、大样本的随机对照研究等。总之,临床施治不应只局限于单一治疗,应根据患者情况制定个性化、综合化的治疗方案,改善患者临床症状,缩短病程。今后,应规范中西医诊疗规范,尽可能选择不良反应少、依赖性弱的药物,开展多中心、大样本的临床试验。Western medicine usually adopts drug treatment, surgical treatment, treatment of meibomian gland to treat xerophthalmia, while traditional Chinese medicine treatments include oral administration of Chinese medicinals based on syndrome differentiation, fumigation of Chinese medicinals, acupuncture treatment, combination of acupuncture and medicine treatment and ultrasonic atomization, which can reduce the patient′s conscious symptoms to varying degrees, increase the secretion of tear and prolong the tear film rupture time.However, the following problems still exist:(1)There is still a lack of standardized diagnosis and treatment standards in the treatment of xerophthalmia, such as the length of treatment, dosage, etc;(2) Western medicine treatment has many adverse reactions, and many patients have certain dependence on drugs;(3) The establishment of classified treatment plan of integrated traditional Chinese and western medicine is not reasonable;(4) There is a lack of long-term multicenter, large sample randomized controlled research in clinical practice.In a word, clinical treatment should not be limited to a single treatment, but should develop a personalized and comprehensive treatment plan according to the patient′s situation to reduce the patient′s pain and shorten the course of the disease.In the future, it is necessary to standardize the diagnosis and treatment of traditional Chinese and western medicine, select drugs with less adverse reactions and weak drug dependence as far as possible, and carry out multi-center and large sample clinical trials.
