摘 要:目的分析同型半胱氨酸(Hcy)、胱抑素C(CysC)、视黄醇结合蛋白(RBP)联合检验在早期诊断糖尿病肾病中的临床价值。方法80例糖尿病肾病患者,以24 h尿蛋白定量不同分为A组(<150 mg/24 h)和B组(150~300 mg/24 h),每组40例;选择同期入院进行体检的40例健康者作为对照组。比较三组研究对象RBP、CysC以及Hcy水平,A组与B组患者三项指标单一检验与联合检验的阳性率。结果A组RBP、CysC、Hcy水平分别为(61.3±6.7)mg/L、(1.1±0.3)mg/L、(13.2±3.3)μmol/L,B组分别为(92.4±8.9)mg/L、(2.4±0.5)mg/L、(18.1±3.6)μmol/L,对照组分别为(42.3±5.4)mg/L、(0.7±0.2)mg/L、(9.0±2.3)μmol/L。B组RBP、CysC、Hcy水平高于A组和对照组,A组高于对照组,三组比较差异具有统计学意义(P<0.05)。A组、B组患者RBP、CysC、Hcy三项联合检验的阳性率均高于同组单一检验,差异具有统计学意义(P<0.05)。B组三项联合检测的阳性率高达100.0%。结论为糖尿病患者联合检验RBP、Hcy和CysC,有助于尽早发现糖尿病肾病,为及时干预预防和后续治疗提供保障。Objective To analyze the clinical value of combined test of homocysteine(Hcy), Cystatin C(CysC) and retinol binding protein(RBP) in the early diagnosis of diabetic nephropathy. Methods A total of 80 patients with diabetic nephropathy were divided into group A(<150 mg/24 h) and group B(150-300 mg/24 h)according to 24-h urinary protein quantification, with 40 patients in each group;40 healthy people who were admitted to the hospital for physical examination at the same time were selected as the control group. Comparison was made on the levels of RBP, CysC and Hcy in the three groups, as well as the positive rates of single test and combined test of three indicators in group A and group B. Results The levels of RBP, CysC, and Hcy were(61.3±6.7) mg/L,(1.1±0.3) mg/L, and(13.2±3.3) μmol/L in group A, and(92.4±8.9) mg/L,(2.4±0.5) mg/L,and(18.1±3.6) μmol/L in group B, and(42.3±5.4) mg/L,(0.7±0.2) mg/L, and(9.0±2.3) μmol/L in the control group. The levels of RBP, CysC, and Hcy in group B were higher than those in group A and the control group, and group A was higher than the control group, and the difference among the three groups was statistically significant(P<0.05). The positive rate of the combined detection of RBP, CysC and Hcy of group A and group B was higher than that of the single detection in the same group, and the difference was statistically significant(P<0.05). The positive rate of co-detection of three indexes in group B was as high as 100.0%. Conclusion Combined test of RBP, CysC, and Hcy for diabetic nephropathy helps to detect diabetic nephropathy as early as possible, providing timely intervention for prevention and follow-up treatment.
