低分子肝素联合乙酰半胱氨酸治疗间质性肺炎患者的疗效观察

摘　　要：目的 观察低分子肝素联合乙酰半胱氨酸治疗间质性肺炎的临床效果。方法 78例间质性肺炎患者为研究对象,依据随机数字表法将其分为参照组与试验组,各39例。参照组予以醋酸泼尼松片联合环磷酰胺治疗,试验组在参照组基础上予以低分子肝素联合乙酰半胱氨酸治疗。比较两组治疗前后血清Ⅲ型胶原蛋白水平、高分辨率CT(HRCT)评分及不良反应发生情况。结果 治疗后,试验组血清Ⅲ型胶原蛋白(60.08±4.34)ng/ml、HRCT评分(14.62±3.91)分均低于参照组的(71.04±5.56)ng/ml、(17.85±3.09)分,差异均有统计学意义(P<0.05)。试验组不良反应发生率为7.69%,参照组不良反应发生率为5.13%,两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论 在常规治疗基础上予以间质性肺炎患者低分子肝素联合乙酰半胱氨酸治疗优势显著,能够改善肺间质纤维化、减轻病变,安全性高。Objective To observe the clinical effect of low molecular weight heparin combined with acetylcysteine in the treatment of patients with interstitial pneumonia. Methods A total of 78 patients with interstitial pneumonia were studied and divided into the reference group and the experimental group according to the random numerical table, with 39 patients in each group. The reference group was treated with prednisone acetate tablets combined with cyclophosphamide, and the experimental group was treated with low molecular weight heparin combined with acetylcysteine on the basis of the reference group. The levels of serum type Ⅲcollagen, high resolution CT(HRCT) score before and after treatment, and occurrence of adverse reactions were compared between the two groups. Results After treatment, the serum type Ⅲ collagen(60.08±4.34) ng/ml and HRCT score(14.62±3.91) points of the experimental group were lower than(71.04±5.56) ng/ml and(17.85±3.09) points of the reference group, and the differences were statistically significant(P<0.05). The incidence of adverse reactions was 7.69% in the experimental group and 5.13% in the reference group. There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05). Conclusion On the basis of conventional treatment, the treatment of low molecular weight heparin and acetylcysteine for patients with interstitial pneumonia has significant advantages, which can improve pulmonary interstitial fibrosis and reduce pathological changes with high safety.
