从“肝玄府到脑玄府”论治脑出血后脑水肿

摘　　要：脑出血是具有较高发病率、致残率和病死率的一种疾病,严重危害人类的健康。脑出血后脑水肿临床上病情复杂多变,可加重原发脑出血疾病。因此,积极控制脑水肿对脑出血的治疗及康复具有重要意义。该文根据脑出血患者脑水肿的临床表现,提出从肝玄府到脑玄府,依据“气津血宜宣通”的治疗原则,采用疏肝行气通窍、化痰祛瘀清脑、质重潜降镇魂、养肝柔肝缓急等治法对脑出血后脑水肿的证治规律进行探究,以期为脑出血后脑水肿的临床治疗提供思路。Intracerebral hemorrhage (ICH) is a severe disease with a high incidence,mortality and disability rate to people’s health.Brain edema,which is difficult to diagnose in clinical practice because of its complexity and variability.The management of cerebral edema is of great significance for the treatment and rehabilitation for ICH.The article was essentially concerned with the thoughts of“from liver mysterious mansion to brain mysterious mansion”for the treatment of brain edema according to the clinical characterization of patients with Brain edema after ICH.Therapeutic methods for reducing brain edema after ICH are:course the liver,move Qi,and free theorizes;transform phlegm and disperse stasis,and clear brain;heavy in substance,subdue and settle the spirit;nourish and emolliate the liver,relax tension.The principle of the treatment is based on“perfusing Qi,fluid and blood”.The main goal of this article is to provide new methods for clinical practice of brain edema after ICH.
