芪术扶正方应用于进展期胃癌的真实世界研究

摘　　要：目的:根据真实世界研究理念,应用该院电子病例系统,观察芪术扶正方联合化疗治疗进展期胃癌的临床研究。方法:以真实世界研究理念下,采用该院完整电子病例系统搜集口服芪术扶正方联合化疗的进展期胃癌、不联合口服中药化疗的进展期胃癌患者。采用病例-对照研究,记录治疗前后中医症状;应用条件Logistic回归分析,分析芪术扶正方联合化疗治疗进展期胃癌的中医症状改善程度,并在研究周期内观察患者无疾病进展时间,利用Kaplan-Meier方法,分析对于无疾病进展时间影响最大客观因素。结果:经多变量Logistic回归分析芪术扶正方对于患者食少、纳呆及腹胀症状缓解明显(p<0.05),体倦乏力及大便性状改变影响小(P>0.05);经Kaplan-Meier方法分析出研究周期内芪术扶正方联合化疗能明显延长胃癌患者无疾病进展时间(P<0.01)。结论:在真实世界研究中发现芪术扶正方能明显改善进展期胃癌食少、纳呆及腹胀症状,提高患者生活质量,并能明显延长患者无疾病进展时间。Objective: To observe the clinical research of Qizhu Fuzheng Prescription combined with chemotherapy in the treatment of advanced gastric cancer by using the electronic case system of the hospital according to the concept of real-world research. Methods: Under the concept of real-world research, the hospital’s complete electronic case system was used to collect patients with advanced gastric cancer who received oral Qizhu Fuzheng Recipe combined with chemotherapy, and patients with advanced gastric cancer who were not combined with oral Chinese medicine chemotherapy. A case-control study was used to record TCM symptoms before and after treatment;conditional Logistic regression analysis was used to analyze the degree of TCM symptom improvement of Qishu Fuzheng Recipe combined with chemotherapy in the treatment of advanced gastric cancer, and the disease progression-free time of patients was observed during the study period. Kaplan-The Meier method was used to analyze the objective factors that had the greatest impact on the time to progression-free disease. Results: According to the multivariate Logistic regression analysis, Qizhu Fuzheng Recipe significantly relieved the symptoms of lack of appetite, anorexia and abdominal distension(P<0.05), but had little effect on fatigue and stool changes(P>0.05);Kaplan-Meier method It was analyzed that Qizhu Fuzheng Prescription combined with chemotherapy during the study period could significantly prolong the progression-free time of gastric cancer patients(P<0.01). Conclusion: In the real-world research, it was found that Qizhu Fuzheng Prescription can significantly improve the symptoms of poor appetite, anorexia and abdominal distension in advanced gastric cancer, improve the quality of life of patients, and significantly prolong the time of patients without disease progression.
