摘  要:目的 探究胎儿颈项透明层(nuchal translucency,NT)厚度联合母体血浆微小RNA-22-3p(micro RNA-22-3p,miR-22-3p)、受体相互作用丝氨酸/苏氨酸蛋白激酶1(receptor-interacting serine/threonine protein kinase 1,RIPK1)对胎儿先天性心脏病(congenital heart disease,CHD)的诊断价值。方法 选择2018年3月至2020年10月在北部战区总医院产科确诊怀有CHD胎儿的孕妇57例为观察对象(CHD组),选择同期怀有健康胎儿的孕妇60名为对照组。采用实时荧光定量聚合酶链反应(polymerase chain reaction,PCR)检测孕妇血浆中miR-22-3p浓度,酶联免疫吸附法检测孕妇血浆中RIPK1浓度,超声检查胎儿NT厚度,Pearson法分析miR-22-3p、RIPK1的相关性,受试者工作特征曲线(receiver operating characteristic curve,ROC)分析胎儿NT厚度联合血浆miR-22-3p、RIPK1对胎儿CHD的预测价值。结果 两组孕妇年龄、孕周及胎儿性别之间比较,差异无统计学意义(P>0.05);与对照组[(2.14±0.32)mm、(2.23±0.54)ng/mL、1.05±0.14]比较,CHD组胎儿NT厚度[(2.93±0.45)mm]增加,血浆RIPK1[(5.68±1.73)ng/mL]浓度显著升高,miR-22-3p(0.67±0.10)浓度显著降低,差异均有统计学意义(P<0.05);相关性分析显示,CHD组血浆RIPK1与miR-22-3p浓度呈负相关性(r=-0.448,P<0.05);NT厚度、血浆RIPK1、miR-22-3p联合预测胎儿CHD的敏感度为89.47%,特异度为85.00%,曲线下面积为0.933。结论 CHD胎儿NT厚度增加,母体血浆中miR-22-3p浓度降低,RIPK1浓度升高;胎儿NT厚度联合母体血浆miR-22-3p、RIPK1浓度检测对胎儿CHD具有一定诊断价值。Objectives To explore the diagnostic value of fetal nuchal translucency(NT)thickness combined with maternal plasma microRNA-22-3p(miR-22-3p)and receptor-interacting serine/threonine protein kinase 1(RIPK1)on fetal congenital heart disease(CHD). Methods From March 2018 to October 2020,57 pregnant women who were diagnosed with CHD fetuses in the Obstetric Department of General Hospital of Northern War Zone were selected as the observation objects(CHD group),and 60 pregnant women with healthy fetuses during the same period were selected as control group. Real-time fluorescent quantitative polymerase chain reaction(PCR)was used to detect the expression level of miR-22-3p in the plasma of pregnant women. Enzyme-linked immunosorbent assay was used to detect the expression level of RIPK1 in the plasma of pregnant women. Ultrasound was used to examine the fetal NT thickness.Pearson method was used to analyze the correlation between miR-22-3p and RIPK1. Receiver operator characteristic curves(ROC)was used to analyze the predictive value of fetal NT thickness combined with plasma miR-22-3p and RIPK1 on fetal congenital heart disease. Results There were no statistically significant differences between the two groups in maternal age,gestational age and fetal sex(P>0.05). Compared with control group[(2.14±0.32)mm,(2.23±0.54)ng/mL,1.05±0.14],the fetal NT thickness[(2.93±0.45)mm]increased,the plasma expression level RIPK1[(5.68±1.73)ng/mL]significantly increased and the miR-22-3p expression level(0.67±0.10)significantly decreased in CHD group,the differences were statistically significant(P<0.05). Correlation analysis showed that plasma RIPK1and miR-22-3p expression levels in CHD group negatively correlated(r=-0.448,P<0.05). The sensitivity of NT thickness,plasma RIPK1 and miR-22-3p in predicting fetal congenital heart disease was 89.47%,specificity was 85.00%,and the area under the curve was 0.933.
