摘 要:目的 探讨颅骨修补术后修补材料外露的特点及再次修复的手术治疗方法。方法 回顾性分析中国康复研究中心北京博爱医院神经外科2012年1月—2020年12月收治的颅骨修补材料外露病例13例,其中钛网修补材料外露10例,聚醚醚酮(PEEK)修补材料外露3例,发生修补材料外露距颅骨修补手术间隔时间1~48个月。结果 本组13例中3例修补材料完全去除,10例修补材料保留。保留修补材料的病例(钛网材料外露9例,PEEK材料外露1例),采用皮瓣松解、筋膜瓣或扩张皮瓣转移等手术方式治疗,效果良好,随访6个月无复发。结论 经临床规范治疗,颅骨修补术后外露修补材料的保留是可行的,采用筋膜瓣转移或扩张皮瓣转移是有效的手术治疗方式。Objective To investigate the features of repair material exposure after cranioplasty and the surgical treatment method for repair again. Methods A retrospective analysis was performed for the clinical data of 13 patients with repair material exposure after cranioplasty who were admitted to Department of Neurosurgery in Beijing Boai Hospital, China Rehabilitation Research Center, from January 2012 to December 2020, among whom 10 had titanium mesh exposure and 3 had polyetheretherketone(PEEK) material exposure, and the interval between repair material exposure and cranioplasty ranged from 1 to 48 months. Results For these 13 patients, the repair material was completely removed for 3 patients and was successfully retained for 10 patients. Among the 10 patients with successfully retained repair material, 9 had titanium mesh exposure and 1 had PEEK exposure, and after surgical treatment such as local flap release, fascial flap transfer, and expanded flap transfer, the patients had a satisfactory treatment outcome with no recurrence after 6 months of follow-up. Conclusion It is feasible to retain the exposed repair material after cranioplasty through standardized clinical treatment, and fascial flap transfer or expanded flap transfer is an effective surgical procedure.
