参附加味颗粒治疗老年心功能不全阳虚血瘀证疗效观察

摘　　要：目的观察参附加味颗粒对老年心功能不全阳虚血瘀证患者的疗效。方法选取120例阳虚血瘀证老年心功能不全患者,随机分为对照组和试验组各60例,对照组给予常规西医治疗,试验组在对照组常规治疗的基础上加用参附加味颗粒。观察治疗14d后患者N-末端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)、急性生理与慢性健康评分II(acute physiology and chronic health evaluationⅡ,APACHE II)评分及中医症状评分的变化。结果治疗14d后,2组患者NT-proBNP水平均较治疗前下降(P<0.05),试验组低于对照组(P<0.05);APACHE II评分、中医症状评分均改善(P<0.05),试验组改善优于对照组(P<0.05);试验组显效率、有效率明显高于对照组(P<0.05)。结论参附加味颗粒辅助治疗阳虚血瘀证老年心功能不全可明显改善患者APACHE II、中医症候评分,降低NT-proBNP水平,临床疗效良好,值得推广应用。Objective To observe the effect of ShenFuJiaWei granule on elderly patients with Yang deficiency and blood stasis syndrome.Methods A total of 120 elderly patients with Yang deficiency and blood stasis syndrome were randomized into control group and experimental group,with 60 cases in each group.The control group was given conventional western medicine treatment,and the experimental group was given ShenFuJiaWei granule on the basis of conventional treatment.The NT-proBNP level,APACHE II score and TCM symptom score of the two groups were observed after 14 days of treatment.Results After 14 days of treatment,the NT-proBNP level in both groups decreased compared with that before treatment(P<0.05).The curative effect of experimental group was better than that of control group(P<0.05).APACHE II score and TCM symptom score in both groups were significantly improved(P<0.05);The improvement of them in the experimental group was significantly better than that in the control group.The significantly effective and effective rate in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion ShenFuJiaWei granule can significantly improve APACHE II score and TCM syndrome score and decrease the NT-proBNP level in elderly patients with Yang deficiency and blood stasis syndrome of heart insufficiency,which suggests that ShenFuJiaWei granule has better clinical efficacy and is worthy of promotion.
