摘　　要：目的为中医药治疗胸痹心痛病提供有效方案和新思路。方法收集胡晓灵医师治疗胸痹心痛病的有效处方473例,利用中医传承辅助平台系统(V2.5)构建数据库,分析其常用药材及其类别、适应证证型、组合规律,并演化出新处方。结果治疗胸痹心痛病的常用药材有丹参、郁金、醋延胡索等23味,常用药材类别有23类,以活血止痛药、活血调经药、消食药等常见;共诊断证型55种,常见的有痰瘀互结证、气滞血瘀证、心血瘀阻证、气阴两虚证、痰浊闭阻证,且丹参、麸炒枳壳、郁金等18味药材与其治疗关联密切;共演化出10种药材核心组合,其中多见郁金、丹参、醋延胡索、法半夏、醋鸡内金等,并演化得到5个新处方。结论治疗胸痹心痛病从心脾肝三脏着手,处方以活血止痛、清化热痰为主,注重健脾益气、舒肝解郁。该研究可为临床治疗胸痹心痛病提供一定借鉴。Objective To provide effective plans and new ideas for the treatment of chest stuffiness and pains with traditional Chinese medicine(TCM).Methods A total of 473 effective prescriptions of Doctor Hu Xiaoling for the treatment of chest stuffiness and pains were collected to analyze the common medicinal materials,categories and combination rules and evolve new prescriptions through the database constructed by the TCM Inheritance Support System(TCMISS,V2.5).Results There were 23 commonly used medicinal materials to treat chest stuffiness and pains,such as Salviae Miltiorrhizae Radix et Rhizoma,Curcumae Radix,vinegar-processed Corydalis Rhizoma.The commonly used medicinal materials could be divided into 23 categories,including blood activating and pain-relieving drugs,blood activating and menstruation regulating drugs,and digestive drugs.A total of 55 types of syndromes were diagnosed,including phlegm and blood stasis syndrome,Qi stagnation and blood stasis syndrome,heart-blood stasis syndrome,deficiency of both Qi and Yin syndrome,phlegm stagnation syndrome,and 18 herbs such as Salviae Miltiorrhizae Radix et Rhizoma,bran-fried Aurantii Fructus,and Curcumae Radix were closely related to the treatment of the above syndromes.Ten core combinations of medicinal materials have evolved,including Curcumae Radix,Salviae Miltiorrhizae Radix et Rhizoma,vinegar-processed Corydalis Rhizoma,Pinelliae Rhizoma Praeparatum,and vinegar-processed Galli Gigeriae Endothelium Corneum,and five new prescriptions have been evolved.Conclusion The treatment of chest stuffiness and pains starts from the heart,spleen and liver.The prescription mainly focuses on promoting blood circulation and relieving pain,clearing hot phlegm,and paying attention to strengthening the spleen and replenishing Qi,relieving the liver and relieving depression.This study can provide a reference for the clinical treatment of chest stuffiness and pains.
