摘　　要：目的:探讨分娩球联合自由体位在瘢痕子宫产妇阴道分娩中的应用效果。方法:选择2019年1月至2021年6月分娩的80例瘢痕子宫再次妊娠拟行阴道试产的产妇,根据随机数字表法分为观察组和对照组各40例,对照组采用传统分娩模式,观察组采用分娩球联合自由体位,观察比较两组产程时间、自然分娩率、产后2h出血量、疼痛程度[采用面部表情评分法(FPS)]、新生儿1 min Apgar评分及窒息情况。结果:观察组第一产程、第二产程、第三产程时间均短于对照组,自然分娩率高于对照组,产后2 h出血量、疼痛程度评分均少于对照组,且差异均有统计学意义(P<0.05);然而,新生儿1 min Apgar评分和窒息率差异无统计学意义(P>0.05)。结论:分娩球联合自由体位能缩短产程,提高剖宫产术后瘢痕子宫阴道自然分娩率,减少分娩疼痛和产后出血量,改善分娩结局。Objective: To investigate the effect of delivery ball combined with free body position in vaginal delivery of women with scar uterus. Methods: 80 parturients who delivered from January 2019 to June 2021 with cicatricial uterus were selected and divided into the observation group and the control group according to the random number table. The control group used the traditional delivery mode.The observation group used the delivery ball combined with the free body position to observe and compare the delivery process time, the rate of natural delivery, the amount of bleeding within 2 hours after delivery, and the degree of pain between the two groups [using the facial expression score(FPS)] Apgar score at 1 minute and asphyxia of newborn. Results: The time of the first stage of labor, the second stage of labor and the third stage of labor in the observation group were shorter than those in the control group, the natural delivery rate was higher than that in the control group, the blood loss and pain score at 2 hours after delivery were lower than those in the control group, and the differences were statistically significant(P<0.05). However, there was no significant difference in 1-minute Apgar score and asphyxia rate of neonates(P>0.05). Conclusion: The delivery ball combined with free body position can shorten the labor process, improve the natural delivery rate of scar uterus and vagina after cesarean section, reduce labor pain and postpartum hemorrhage, and improve the delivery outcome.
