摘 要:目的探讨生物羊膜在手外科断指再植术中的应用效果。方法60例接受手部断指再植术患者为研究对象,按照随机数字表法分为对照组和实验组,每组30例。对照组采用断指再植术治疗,实验组在对照组基础上联合生物羊膜治疗。比较两组术后2、4个月手指总主动活动度(TAM)、断指成活率及肌腱、神经血管粘连情况。结果实验组术后2、4个月手指TAM分别为(185.2±10.6)、(221.4±12.2)°,大于对照组的(174.5±9.7)°、(205.5±15.8)°,差异有统计学意义(P<0.05)。治疗10 d后,两组断指成活率比较差异无统计学意义(P>0.05);实验组肌腱粘连率和神经粘连率分别为3.33%、6.67%,低于对照组的20.00%、26.67%,差异有统计学意义(P<0.05)。结论断指再植术中应用生物羊膜治疗不影响断指成活率,且能有效防止肌腱和神经血管粘连,值得临床推广应用。Objective To discuss the application effect of biological amnion in replantation of severed fingers in hand surgery. Methods A total of 60 patients who received replantation of severed fingers of the hand were selected as the research subjects, and the patients were divided into the control group and the experimental group according to the random numerical table, with 30 patients in each group. The control group was treated with replantation of severed fingers, and the experimental group was treated with biological amnion on the basis of the control group. The total active movement(TAM) of the fingers 2 and 4 months after surgery, the survival rate of severed fingers, and the adhesion of tendons, nerves and blood vessels were compared between the two groups.Results The TAM of the finger at 2 and 4 months after surgery in the experimental group were(185.2±10.6)and(221.4±12.2)°, which were greater than(174.5±9.7) and(205.5±15.8)° in the control group, and the differences were statistically significant(P<0.05). After 10 d of treatment, there was no statistically significant difference in the survival rate of severed fingers between the two groups(P>0.05). The tendon adhesion rate and nerve adhesion rate in the experimental group were 3.33% and 6.67%, which were lower than 20.00% and 26.67%in the control group, and the differences were statistically significant(P<0.05). Conclusion The application of biological amnion in replantation of severed fingers does not affect the survival rate of severed fingers, and can effectively prevent tendon and neurovascular adhesion, which is worthy of clinical promotion and application.
