学龄前儿童中重度阻塞性睡眠呼吸暂停患儿不同术式疗效分析

摘　　要：目的 比较学龄前儿童中重度阻塞性睡眠呼吸暂停(OSA)患儿不同术式疗效。方法 选择2020年8月—2021年2月收治的学龄前中重度阻塞性睡眠呼吸暂停(OSA)手术患儿124例作为研究对象,回顾性分析124例手术患儿,按照术式不同分4组,A组(低温等离子腺样体切除),B组(低温等离子腺样体切除联合双扁桃体部分切除),C组(鼻切削钻腺样体切除),D组(鼻切削钻腺样体切除联合双扁桃体部分切除),各组均为31例,比较四种术式的疗效。结果术后12个月,4组患儿免疫指标、睡眠呼吸暂停低通气指数(AHI)、睡眠时最低血氧、OSA-18生活质量问卷得分比较差异均无统计学意义(P>0.05);4组患儿术后不良反应比较差异无统计学意义(P>0.05)。结论 学龄前中重度阻塞性睡眠呼吸暂停(OSA)患儿采用低温等离子切除和鼻切削钻切除疗效一致。Objective To compare the efficacy of different surgical methods for preschool children with moderate to severe obstructive sleep apnea(OSA).Methods A total of 124 preschool children with moderate to severe obstructive sleep apnea(OSA) were selected in our hospital from August 2020 to February 2021,retrospective analysis of 124 surgical children, they were divided into 4 groups according to different surgical methods, group A(low-temperature plasma adenoidectomy) and group B(low-temperature plasma adenoidectomy combined with partial tonsillectomy),group C(nasal dynamical system adenoidectomy),group D(nasal dynamical system adenoidectomy combined with partial tonsillectomy),31 cases in each group, the effects of the four surgical methods were compared.Results At 12 months after surgery, there were no statistically significant differences in immune indexes, apnea-hypopnea index(AHI),lowest level of blood oxygen saturation during sleeping period at night and the total score of OSA-18 life questionnaire among the 4 groups(P>0.05).There was no statistically significant difference in adverse reactions(P>0.05).Conclusion Low-temperature plasma and nasal dynamical system surgical treatment both had good clinical efficacy for preschool children with moderate to severe obstructive sleep apnea(OSA).
