高低级别胶质瘤中包含Jumonji C结构域蛋白6表达的差异及其与预后的关系

摘　　要：目的:探讨包含Jumonji C结构域蛋白6(JMJD6)在高低级别胶质瘤中表达的差异及其与胶质瘤预后的关系。方法:从2018年1月至2020年12月广东省人民医院神经外科收治的胶质瘤患者中随机抽取42例肿瘤标本,分为低级别组和高级别组,其中,低级别18例、高级别24例;用免疫组织化学法检测两组JMJD6的表达并采用t检验和χ^(2)检验比较两组JMJD6表达水平的差异;随访患者术后肿瘤复发及患者死亡,按JMJD6表达水平(去除阴性病例),分为低JMJD6表达组和高JMJD6表达组,随访40个月,采用χ^(2)检验、Kaplan-Meier曲线和Pearson相关分析JMJD6表达水平与预后的关系。结果:低级别组JMJD6的表达呈阴性7例、低表达9例、高表达2例;高级别组阴性0例、低表达9例、高表达15例;低级别胶质瘤JMJD6表达水平(2.06±2.88)与高级别胶质瘤JMJD6表达水平(7.88±1.90)比较,差异有统计学意义(t=-7.449,P<0.01)。低JMJD6表达组18例,高JMJD6表达组17例;低JMJD6表达组复发7例,复发率38.89%,死亡病6例,死亡率33.33%;高JMJD6表达组复发14例,复发率82.35%,死亡13例,死亡率76.47%;两组复发率及死亡率比较,差异均有统计学意义(χ^(2)=6.882,P<0.01和χ^(2)=6.556,P<0.05);JMJD6的表达水平与复发率及死亡率呈正相关(r=0.626、0.659,P<0.001);与低JMJD6表达组比较,高JMJD6表达组的患者生存时间更短(χ^(2)=7.631,P<0.01)。结论:胶质瘤级别越高,JMJD6表达水平也越高,JMJD6表达水平越高,肿瘤越易复发,患者预后越差,JMJD6有助于预测肿瘤复发及预后。Objective To investigate the expression of Jumonji C-domain containing protein 6(JMJD6)in high and low grade gliomas and its relationship with prognosis of glioma.Methods 42 cases of glioma patients which admitted to our department from 2018 to 2020 were randomly selected and divided into low-grade group and high-grade group,including 18 cases of low-grade and 24 cases of high-grade.The expression of JMJD6 was detected by immunohistochemistry and the difference of JMJD6 expression between the two groups was compared by t-test and Chi-square test.According to the expression level of JMJD6(excluding negative cases),the patients were divided into low JMJD6 expression group and high JMJD6 expression group.The relationship between JMJD6 expression level and prognosis was analyzed by Chi-square test,Kaplan-Meier curve and Pearson correlative analysis.Results The expression of JMJD6 in low grade group was negative in 7 cases,low expression in 9 cases and high expression in 2 cases.In the high-grade group,0 cases were negative,9 cases were low expression,and 15 cases were high expression.The expression level of JMJD6 in high-grade glioma(7.88±1.90)was significantly higher than that in low-grade glioma(2.06±2.88,t=-7.449,P<0.01).There were 18 cases in low JMJD6 expression group and 17 cases in high JMJD6 expression group.In the low JMJD6 expression group,7 cases(38.89%)recurried and 6 cases(33.33%)died.In the high JMJD6 expression group,14 cases had recurrence,the recurrence rate was 82.35%,and 13 cases died,the mortality rate was 76.47%.There were significant differences in the recurrence rate and death rate between the two groups(χ^(2)=6.882,P<0.01;χ^(2)=6.556,P<0.05).The expression level of JMJD6 was positively correlated with the recurrence rate and mortality rate(r=0.626,0.659,respectively,P<0.001).Compared with the low JMJD6 expression group,the overall survival of patients in the high JMJD6 expression group was more shorter(χ^(2)=7.631,P<0.01).Conclusion The higher grade of glioma,the higher the expression lev
