摘  要:目的 薏苡仁油(康莱特注射液)在临床上广泛用于减轻非小细胞肺癌的放化疗副反应,其机制尚不明确。文章通过60Coγ射线辐照小鼠胸部建立放射性肺炎模型,采取薏苡仁油对模型小鼠腹腔注射的方式,观察薏苡仁油对胸部照射所致放射性肺炎的防治作用。方法 将54只C57BL/6J小鼠,随机分为3组,正常对照组、辐照组和薏苡仁油组,每组18只,依据取材时间不同,分为2、4和8周3个时间段,每个时间段6只。用60Coγ射线对小鼠胸部进行辐照,单次剂量25 Gy,建立放射性肺炎模型。辐照组、薏苡仁油组于辐照后2、4和8周对小鼠进行处死取血,检测其外周血白细胞数。同时段取肺组织进行HE染色,观察其肺部病理。采用酶联免疫吸附法(ELISA)检测小鼠肺组织细胞因子:白细胞介素-1β(IL-1β)、IL-6、IL-18和肿瘤坏死因子-α(TNF-α)。结果 薏苡仁油能显著抑制辐照导致的小鼠外周血白细胞数量降低,阻止辐照导致的白细胞下降,且这种对白细胞的保护作用,在辐照后4周最为显著,与辐照组白细胞相比(P<0.01);薏苡仁油能改善小鼠肺部损伤,减轻肺部炎性渗出,维持肺泡结构的完整性,在辐照后4周效果最为显著;薏苡仁油能够调节辐照后肺部炎症因子的表达,这种调节作用在辐照后4周效果最为显著。4周时薏苡仁油组IL-1β[259.70±35.65]、IL-6[204.90±24.46]、IL-18[316.70±21.81]和TNF-α[1304.00±114.10]pg/g,同时段辐照组IL-1β[320.50±45.27]、IL-6[274.90±42.86]、IL-18[435.40±17.98]、TNF-α[1724.00±259.2]pg/g,两者相比薏苡仁油组炎症因子表达显著降低(P<0.05)。结论 薏苡仁油能够改善胸部照射导致的小鼠外周血象变化,抑制辐照导致白细胞急剧减少。减轻肺部损伤,降低肺组织炎症因子的表达,是潜在的放射性肺炎防治药物。Objective Coix seed oil(Kanglaite injection) is widely used clinically to reduce the side effects of radiotherapy and chemotherapy in non-small cell lung cancer, but its preventative and therapeutical role of radioactive lung injury is still unclear. To establish a radiation-induced pneumonia model in the chest of mice irradiated with 60Co γ-rays. Coix seed oil was injected intraperitoneally into the model mice to observe the prevention and treatment effect of Coix seed oil on radiation-induced pneumonia caused by chest irradiation. Methods A total of 54 C57BL/6J mice were randomly divided into 3 groups: normal control group, irradiation group and coix seed oil group, with 18 mice in each group. According to different sampling time, they were divided into three periods, with 6 mice in each period. The mouse chest was irradiated with 60Co γ-rays at a single dose of 25Gy to establish radiation pneumonitis model. Mice in the irradiation group and coix seed oil group were sacrificed at 2, 4 and 8 weeks after irradiation and blood was collected to detect the number of white blood cells in their peripheral blood. At the same time, the lung tissues were collected for HE staining to observe the pathology of lungs. Interleukin-1β(IL-1β), interleukin-6(IL-6), interleukin-18(IL-18) and tumor necrosis factor-α(TNF-α) were detected by enzyme-linked immunosorbent assay(ELISA). Results Coix seed oil significantly inhibited the reduction of leukocytes in peripheral blood of mice caused by irradiation and prevented the reduction of leukocytes caused by irradiation. This protective effect on leukocytes was most significant at 4 weeks after irradiation, compared with that of the irradiation group(P<0.01). Coix seed oil improved the lung injury of mice, reduced pulmonary inflammatory exudation and maintained the integrity of alveolar structure. And the most significant effect was 4 weeks after irradiation. Coix seed oil regulated the expression of inflammatory factors in the lungs after irradiation. And this regulation effect
