摘 要:目的探讨血小板与淋巴细胞比值(platelet-to-lymphocyte ratio,PLR)和红细胞分布宽度(red blood cell distribution width,RDW)与脑梗死(cerebral infarction,CI)的相关性及其临床意义。方法收集2019年1月至2020年1月就诊于盘锦市中心医院神经内科的CI患者190例,健康体检者100例作为对照组。收集一般临床资料,测定血常规等相关指标。根据NIHSS评分,将CI患者分为轻型、中型和重型3个亚组,比较不同亚组间外周血PLR和RDW水平。采用Spearman相关分析分析PLR和RDW与CI严重程度的相关性,并采用ROC曲线评价PLR和RDW在CI发生发展中的预测价值。结果(1)研究组患者外周血PLR(169.00±31.37)%和RDW(14.31±2.06)%水平均高于对照组,差异具有统计学意义(P<0.05);(2)研究组各亚组之间组间RDW[轻型(137.58±18.45)%、中型(152.92±20.53)%、重型(176.21±22.35)%]存在差异,差异具有统计学意义;(3)研究组患者的PLR与NIHSS呈正相关(r=0.604,P<0.001),RDW与NIHSS呈正相关(r=0.583,P<0.001),RDW与PLR呈正相关(r=0.566,P<0.001);(4)PLR和RDW预测CI患者病情严重程度的敏感度分别为82.3%、85.8%,特异度分别为64.7%、79.8%,联合预测的敏感度为85.8%、特异度为94.3%。结论PLR和RDW在CI的发生和发展过程中起到重要作用,对CI严重程度的评估具有一定的预测作用。Objective To investigate the correlation of platelet to lymphocyte ratio(PLR)and red blood cell distribution width(RDW)with cerebral infarction(CI).Methods A total of 190 CI patients who were treated in the Department of Neurology of Panjin Central Hospital from January 2019 to January 2020,and 100 healthy subjects were collected as the control group.General clinical data,measure blood routine and other related indicators were collected.According to the NIHSS score,patients with CI were divided into three subgroups:mild,moderate and severe,and the levels of PLR and RDW in peripheral blood were compared between different subgroups.Spearman correlation analysis was used to analyze the correlation between PLR and RDW and the severity of CI,and ROC curve was used to evaluate the predictive value of PLR and RDW in the occurrence and development of CI.Results(1)The levels of peripheral blood PLR(169.00±31.37)%and RDW(14.31±2.06)%in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05);(2)there were differences in RDW(light(137.58±18.45)%,medium(152.92±20.53)%,and severe(176.21±22.35)%)in the study group,and the difference was statistically significant;(3)in the study group,PLR was positively correlated with NIHSS(r=0.604,P<0.001),RDW was positively correlated with NIHSS(r=0.583,P<0.001),and RDW was positively correlated with PLR(r=0.566,P<0.001);(4)the sensitivity of PLR and RDW to predict the severity of CI patients was 82.3%and 85.8%respectively,and the specificity was 64.7%and 79.8%respectively.The sensitivity of combined prediction was 85.8%and the specificity was 94.3%.Conclusion PLR and RDW play an important role in the occurrence and development of CI,and can predict the severity of CI.
