曲霉IgG抗体检测对慢性肺曲霉病的诊断价值

摘　　要：目的研究曲霉免疫球蛋白G(IgG)抗体检测对慢性肺曲霉病(CPA)的诊断价值。方法选择2020年4月至2022年3月于赣州市人民医院治疗的82例疑为CPA患者,以肺部病灶组织病理检查到曲霉菌为金标准,分为CPA组(45例)和非CPA组(37例)。采集患者清晨空腹静脉血5 ml,采用酶联免疫吸附法测定曲霉IgG抗体检测。比较两组曲霉IgG抗体检测结果,采用ROC曲线评价曲霉IgG抗体对CPA的诊断效能,计算ROC曲线下面积(AUC),计算曲霉IgG抗体诊断敏感度及特异度。结果CPA组曲霉IgG抗体水平为(148.76±37.19)AU/ml,高于非CPA组的(58.67±15.24)AU/ml,差异有统计学意义(P<0.05);曲霉IgG抗体以120 AU/ml为临界值,曲霉IgG抗体检测诊断CPA的AUC为0.783(95%CI:0.681~0.886),诊断敏感度为65.46%,诊断特异度为73.68%。结论CPA患者的曲霉IgG抗体水平升高,检测曲霉IgG抗体对CPA具有一定的诊断性,诊断敏感度及特异度较高,能够为早期诊断CPA与及时采取治疗措施提供指导。Objective To study the diagnostic value of Aspergillus immunoglobulin G(IgG)antibody in chronic pulmonary aspergillosis(CPA).Methods A total of 82 patients with suspected CPA who were treated in Ganzhou People's Hospital from April 2020 to March 2022 were selected and divided into CPA group(45 cases)and non-CPA group(37 cases)according to the gold standard of Aspergillus detected by histopathological examination of lung lesions.5 ml of fasting venous blood was collected from patients in the morning,and Aspergillus IgG antibody was detected by enzyme-linked immunosorbent assay.The detection results of Aspergillus IgG antibodies between the two groups were compared,and the diagnostic results of Aspergillus IgG antibody on CPA was evaluated by ROC curve,area under curve(AUC)was calculated,and the diagnostic sensitivity and specificity of Aspergillus IgG antibody were calculated.Results The level of Aspergillus IgG antibody in CPA group was(148.76±37.19)AU/ml,which was higher than(58.67±15.24)AU/ml in non-CPA group,and the difference was statistically significant(P<0.05).The critical value of aspergillus IgG antibody was 120 AU/ml,the AUC of Aspergillus IgG antibody detection and diagnosis of CPA was 0.783(95%CI:0.681-0.886),the diagnostic sensitivity was 65.46%,and the diagnostic specificity was 73.68%.Conclusion Aspergillus IgG antibody level is increased in CPA patients.Detection of Aspergillus IgG antibody has certain diagnostic value,high diagnostic sensitivity and specificity for CPA,which can provide guidance for early diagnosis of CPA and timely treatment.
