悬吊运动疗法治疗退行性腰椎滑脱症临床观察

摘　　要：目的观察悬吊运动疗法(SET)对退行性腰椎滑脱症(DLS)患者脊柱-骨盆矢状位参数的影响及康复效果。方法选择2020年1月至2022年3月在我院康复科接受治疗的80例退行性腰椎滑脱症患者为研究对象,按照随机数字表法分为观察组和对照组各40例。观察组患者进行常规康复项目和悬吊运动疗法治疗,对照组应用常规康复项目及中医整脊方法进行干预。治疗前、6w后测量两组患者脊柱-骨盆矢状位参数:矢状面垂直轴(SVA)、胸椎后凸角(TK)、腰椎前凸角(LL)、骨盆入射角(PI)、骨盆倾斜角(PT)及骶骨倾斜角(SS),评价两组患者VAS疼痛评分和腰椎JOA评分。结果6w后,观察组患者SS、LL角度较治疗前显著减小(P<0.01),PT角度、SVA数值较治疗前明显增大(P<0.05),对照组患者仅SS角度较治疗前明显减小(P<0.05)。6w后组间比较,观察组患者SS、LL角度明显小于对照组(P<0.05),PT角度、SVA数值明显大于对照组(P<0.05)。6w后,两组患者VAS疼痛评分较治疗前均显著降低(P<0.01),腰椎JOA评分较治疗前均显著提高(P<0.01),且观察组评分优于对照组(P<0.05)。结论悬吊运动疗法调整退行性腰椎滑脱症患者脊柱-骨盆矢状位参数,改善患者VAS疼痛评分、腰椎JOA评分,疗效优于中医整脊方法。Objective To observe the effect of suspension exercise therapy(SET)on sagittal parameters of spine and pelvis in patients with degenerative lumbar spondylolisthesis(DLS)and its rehabilitation effect.Methods Choose between January 2020 and March 2022 at our hospital rehabilitation treatment of 80 cases of patients with degenerative lumbar olisthe disease as the research object,according to random number table method divided into observation group and control group 40 cases,observation group of patients performing routine rehabilitation project and suspension movement therapy treatment,control group used conventional rehabilitation project and TCM chiropractic methods to intervene.Before and 6 weeks after treatment,thesagittal parameters of the affected spine and pelvis were measured,including sagittal vertical axis(SVA),thoracic kyphosis Angle(TK),lumbar lordo Angle(LL),pelvic incidence Angle(PI),pelvic inclination Angle(PT)and sacral inclination Angle(SS).VAS pain score and lumbar male J0 A score were evaluated.Results After 6 weeks,The SS and LL angles of the observation group were decreased significantly(P<0.01),PT Angle and SVA values were increased significantly(P<0.05).In the control group,only the Angle of SS decreased significantly(P<0.05).After 6 weeks,the SS、LL Angle of the observation group was significantly lower than that of the control group(P<0.05),and the PT Angle and SVA values of the observation group were significantly higher than those of the control group(P<0.05).After 6 weeks,the VAS score of the two groups was significantly lower than that before treatment(P<0.01),and the JOA score of the two groups was significantly higher than that before treatment(P<0.01),and the score of the observation group was better than that of the control group(P<0.05).Conclusion Suspension exercise therapy can adjust the sagittal parameters of spine and pelvis in patients with degenerative lumbar spondylolisthesis,improve VAS pain score and lumbar JOA score,and has better curative effect than traditional Chinese
