失能老年患者“四元联动”护理服务体系的构建与应用研究

摘　　要：目的构建与探索失能老年患者“四元联动”护理服务体系,提升对失能老年患者的全周期康复照护质量。方法选取2021年2月至7月在我院住院且符合入组条件的460例失能老年患者,分为对照组和观察组各230例。对照组采用常规康复护理干预,观察组采用“四元联动”护理服务体系(医院—社区联合护理模式、医养结合护理模式、互联网+护理服务模式)进行干预。对比分析两组患者出院指导质量、社会支持情况、日常生活活动能力(BI评分),评价失能老年患者“四元联动”护理服务体系的实施效果。结果观察组患者出院指导质量量表得分高于对照组(P<0.05);社会支持评定量表入院时无统计学差异(P>0.05),出院时观察组得分高于对照组,差异有统计学意义(P<0.05);两组患者BI评分存在显著的时间效应、组间效应及交互效应,且观察组转介社区、养老院、居家的患者得分均高于对照组。结论失能老年患者“四元联动”护理服务体系的构建与应用能提高失能老年患者出院指导质量和社会支持度,确保患者出院后的后续康复护理质量,促进其日常生活活动能力的恢复或维持。Objective To construct and explore a“four-element linkage”nursing service system for elderly patients with disability,and to improve the quality of full-cycle rehabilitation care for elderly patients with disability.Methods 460 disabled elderly patients who were hospitalized in our hospital from February to July 2021 and met the inclusion conditions were selected and divided into control group and observation group,230 cases in each group.The control group was given routine rehabilitation nursing intervention,and the observation group was given“four-element linkage”nursing service system(hospital-community joint nursing mode,medical-nursing combined nursing mode,Internet+nursing service mode).Quality of discharge guidance and social support of patients,activities of daily living(BI score)in the two groups were compared and analyzed,and the implementation effect of“four-dimensional linkage”nursing service system for elderly patients with disability was evaluated.Results The scores of discharge guidance quality scale in observation group were higher than those in control group(P<0.05).There was no significant difference in the social support rating scale at admission(P>0.05),and the score of the observation group was higher than that of the control group at discharge,with statistical significance(P<0.05).There were significant time effect,intergroup effect and interaction effect in BI scores of patients in the two groups,and the scores of patients in the observation group who were referred to community,nursing home and home were higher than those in the control group.Conclusion The construction and application of“four-component linkage”nursing service system for elderly patients with disability can improve the quality of discharge guidance and social support for elderly patients with disability,ensure the quality of follow-up rehabilitation nursing after discharge,and promote the recovery or maintenance of their daily living activities.
