OSAHS患者悬雍垂腭咽成形术后延迟拔管对术后安全的影响

摘　　要：目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者悬雍垂腭咽成形术(UPPP)后延迟拔管的护理及术后安全问题。方法选择2018年6月—2021年6月就诊于本院的116例OSAHS患者作为研究对象,按照随机数表法分为观察组(65例)和对照组(51例)两组。两组患者均采用UPPP术式手术治疗,观察组术后延迟拔管4-6小时,加强术前指导,术后气道护理、局部观察、口腔护理及生命体征监护,及时处理并发症。对照组麻醉清醒后复苏室拔管,术后加强口腔护理和生命体征观察。比较两组患者治疗效果和并发症发生情况。结果观察组患者的治疗总有效率(100.00%)明显高于对照组(94.12%),差异具有统计学意义(P<0.05);观察组患者术后并发症发生率(16.93%)明显低于对照组(41.17%),差异有统计学意义(P<0.05)。结论UPPP术后延迟拔管可以有效提高治疗效果,减少和避免相关并发症的发生。提高手术安全性,术后气道护理是重要环节。Objective To analyze the nursing and postoperative safety of delayed extubation after uvula palatopharyngoplasty(UPPP)in OSAHS patients.Methods 116 OSAHS patients those were admitted to our hospital during June 2018 and June 2021 were enrolled as study subjects.According ro random number table method they were divided into observation group and control group,and there were 65 patients in the observation group and 51 cases in the control group.All patients were treated with UPPP,and the extubation was delayed for 4-6 hours after surgery in the observation group,preoperative guidance was performed,strengthen postoperative airway care,oral care and vital signs monitoring,complications were treated promptly.In the control group the endotracheal tube was removed in resuscitation room after anesthesia,and oral care and vital signs were observed.The treatment effect and complications were compared between the two groups.Results The total effective rate of the observation group(100.00%)was significantly higher than that of the control group(94.12%),and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group(16.93%)was significantly lower than that in the control group(41.17%),and the difference was statistically significant(P<0.05).Conclusions Delayedextubation after UPPP could effectively reduce and avoid the occurrence of related complications and improve the safety of surgery,postoperative airway nursing is an important link.
