摘　　要：目的探讨叙事医学对老年人脑卒中后阈下抑郁的干预效果。方法选择2020年12月―2021年12月本院神经内科收治的64例老年脑卒中患者作为研究对象,采用随机数表法将患者分为干预组和对照组两组,每组各32例。对照组实施常规护理,干预组在对照组所采取措施的基础上实施叙事医学护理,每周1次,30-40分/次,为期12周。干预前及干预3个月后采用汉密尔顿-17项抑郁量表(HAMD-17)、流调中心抑郁量表(CES-D)评价两组的干预效果。结果干预组,干预3个月后CES-D得分明显高于对照组,差异有显著统计学意义(P<0.01);干预后,两组患者的阈下抑郁转归比较,差异有统计学意义(P<0.05)。结论叙事医学护理能有效降低病程≤3个月老年人脑卒中后阈下抑郁和脑卒中后抑郁的发生率。Objective To investigate the effect of narrative medicine on sub-threshold depression in seniors after cerebral apoplexy.Methods 64 cases of senile patients suffered from cerebral apoplexy those hospitalized in neurology department of our hospital from December 2020 to December 2021 were selected as the research object.They were divided into the intervention group and the control group according to random number table method,32 cases in each group.The control group received routine nursing intervention,and the intervention group received narrative medical nursing intervention on the basis of the control group,once a week,30-40min per time,and lasted for 12 weeks.Hamilton-17-item Depression Scale(HAMD-17)and epidemiological survey center depression scale(CES-D)were used to evaluate the intervention effect of the two groups before intervention and 3 months after intervention.Results After 3 months of intervention,the score of CES-D in the intervention group were significantly higher than that of control group(P<0.01).After intervention,there was statistical significance in the outcome of sub-threshold depression between the two groups(P<0.05).Conclusions Narrative medical nursing could improve the incidence of sub-threshold depression and depression in senile patients suffered from cerebral apoplexy whose disease duration less or equal to 3 months.
