自服农药病人重症监护室治疗期间真实体验的质性研究

摘　　要：目的:调查自主服用农药病人在重症监护室(ICU)治疗期间的真实感受和体验,为临床护理人员开展个性化护理提供依据。方法:采用现象学研究方法对宁波市某综合性三级甲等医院ICU 2020年7月—2021年12月收治的11例自服农药病人进行深入访谈,运用Colaizzi 7步分析法进行资料分析。结果:自服农药病人在ICU治疗期间的真实体验可归纳为4个主题,即生理的痛苦(口咽部疼痛,化学性胃肠炎);自我感觉负担较重(疼痛灾难化,担忧假设性问题,经济负担);生活质量下降(睡眠障碍,饮食限制);渴望帮助和支持(希望自身康复,渴望社会支持,情感支持需求)。结论:护理人员应加强疼痛管理,关注病人的负担和需求,加强多方面的社会支持等,帮助病人解决困难,促进其身心康复。Objective:To investigate the real feeling and experience of patients with self-administered pesticides during intensive care unit(ICU)treatment,and to provide the basis for clinical nursing staff to carry out individualized nursing.Methods:Phenomenological research method was used to conduct in-depth interviews on 11 patients with self-administered pesticides in ICU in a comprehensive third-class A hospital in Ningbo from July 2020 to December 2021,and Colaizzi seven-step analysis method was used for data analysis.Results:The real experience of self-administered pesticide patients during ICU treatment can be summarized into four themes,namely physiological pain(oropharyngeal pain,chemical gastroenteritis);heavy burden of self-perception(pain disaster,worry about hypothetical problems,economic burden);decreased quality of life(sleep disorder,dietary restriction);longing for help and support(hope their own rehabilitation,longing for social support,emotional support needs).Conclusion:Nursing staff should strengthen pain management,pay attention to the burden and needs of patients,strengthen social support and other nursing measures to help patients solve difficulties and promote their physical and mental rehabilitation.
