摘　　要：目的探讨乳酸林格氏液预防内镜下逆行胰胆管造影术(ERCP)发生术后胰腺炎的效果。方法选取2021年1月至12月在上饶市人民医院择期行ERCP的60例患者作为研究对象,采用随机数字表法将其分为对照组(30例)和观察组(30例)。对照组患者术前予以乳酸林格氏液5 ml/(kg·h)静脉滴注,观察组患者在此基础上于术后再静脉滴注乳酸林格氏液20 ml/(kg·h)。比较两组患者的术后腹痛或腹痛加重发生情况;比较两组患者的血清淀粉酶和白细胞计数检测结果;比较两组患者的胰腺炎及容量超负荷发生情况。结果观察组患者的术后腹痛、腹痛加重发生率均低于对照组,差异有统计学意义(P<0.05);观察组患者术后3、24 h的血清淀粉酶、白细胞计数水平均低于对照组,差异有统计学意义(P<0.05);两组患者的胰腺炎发生率、容量超负荷发生率比较,差异无统计学意义(P>0.05)。结论乳酸林格氏液的应用能够有效预防患者术后胰腺炎的发生,且术前、术后均应用乳酸林格氏液更有助于ERCP术后患者临床症状的改善,可更有效地控制淀粉酶、白细胞计数水平。Objective To investigate the effect of Lactated Ringer's Solution in the prevention of pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP).Methods A total of 60 patients who underwent ERCP in Shangrao People's Hospital from January to December 2021 were selected as the study subjects,and the patients were divided into control group(30 cases)and observation group(30 cases)by random number table method.The control group was given 3 ml/(kg·h)of Lactated Ringer's Solution by intravenous drip before operation,and the observation group was given 20 ml/(kg·h)of Lactated Ringer's Solution intravenously after the operation.The occurrence of postoperative abdominal pain or abdominal pain aggravation was compared between the two groups.The results of serum amylase and leukocyte count were compared between the two groups.Pancreatitis and volume overload were compared between the two groups.Results The incidence rates of postoperative abdominal pain and abdominal pain aggravation in observation group were lower than those in control group,the differences were statistically significant(P<0.05).The serum amylase and leukocyte count levels in the observation group were lower than those in the control group at 3 and 24 h after surgery,the differences were statistically significant(P<0.05).There were no significant differences in the incidence rates of pancreatitis and volume overload between the two groups(P>0.05).Conclusion The application of Lactated Ringer's Solution can effectively prevent the occurrence of postoperative pancreatitis in patients,and the application of Lactated Ringer's Solution both before and after surgery is more conducive to the improvement of clinical symptoms in patients after ERCP surgery,and can more effectively control the level of amylase and leukocyte count.
