腹腔镜下采用倒刺线处理根部坏疽阑尾的疗效探讨

摘　　要：目的:探讨腹腔镜阑尾切除术中倒刺线缝合包埋阑尾残端的临床效果。方法:选取2018年1月-2021年12月揭阳市人民医院收治的60例急性根部坏疽阑尾炎患者作为研究对象,采用随机数字表法将其分为对照组和研究组,各30例。对照组采用可吸收线间断缝合包埋法,研究组采用倒刺线连续性浆肌层缝合法包埋阑尾残端。对比两组患者术中和术后相关指标及术后并发症发生率。结果:研究组患者术中出血量少于对照组,住院费用低于对照组,包埋时间、手术时间、肛门排气时间、住院时间均短于对照组(P<0.05),根部包埋成功率高于对照组(P<0.05)。研究组患者肠梗阻、出血、切口感染、腹痛、粪瘘并发症的发生率低于对照组(P<0.05)。结论:腹腔镜阑尾切除术中采用倒刺线缝合包埋阑尾残端临床效果显著,安全性更高,能够有效缩短缝合时间和手术时间,降低手术难度,值得临床推广应用。Objective:To investigate the clinical effect of embedding appendiceal stump with barbed suture during laparoscopic appendectomy.Method:A total of 60 patients with acute root gangrenous appendicitis admitted to the Jieyang People’s Hospital from January 2018 to December 2021 were selected as the research objects,and were divided into control group and study group by random number table method,with 30 cases in each group.In the control group,intermittent suture with absorbable line was used,and in the study group,continuous suture with barb line was used to enclose the appendiceal stump.The intraoperative and postoperative related indexes and the incidence of postoperative complications were compared between the two groups.Result:The intraoperative blood loss in the study group was less than that in the control group,the hospitalization cost was lower than that in the control group,the embedding time,operation time,anal exhaust time and hospital stay were shorter than those in the control group(P<0.05),and the success rate of root embedding was higher than that in the control group(P<0.05).The incidence of intestinal obstruction,bleeding,incision infection,abdominal pain,fecal fistula in the study group was lower than that in the control group(P<0.05).Conclusion:The use of barbed suture to embed the appendiceal stump during laparoscopic appendectomy is clinically effective and safer,which can effectively shorten the suturing time and operation time and reduce the difficulty of the operation,and is worthy of clinical application.
