从中医“男子生殖轴”论治精液异常性不育症

摘　　要：以男子精液质量异常性不育症为目标,查询古代文献中有关男子“天癸”“男子胞”“精窍”“精液”“精薄”“精少”“精清”“精寒”“精热”“精瘀”“精涩”“精浊”和“精滑”记载,建立男子生殖轴,论述各个脏腑与生殖轴中精液生成的关系,依据精液生成的关键环节辨证施治。文章中男子生殖轴为:天癸←→肾→男子胞→精窍→精液,分为精液源头、精液转化和精液排泄与储存3个环节。在精液源头环节应健脾补肾,转化环节应温阳补血,在精液排泄与储存环节应疏肝补肾、宁心健脾。根据影响精液生成的病理因素,可补虚、清热、温阳、化湿和化瘀。文章全面论述男子生殖轴,可对临床治疗男性精液异常所致的不育症提供理论参考与借鉴。Aiming at men’s sperm quality abnormal infertility, query in the ancient literature about man ‘Tian gui’‘Nan zi bao’ ‘Jing qiao’ ‘semen’ ‘Jing bo’ ‘Jing shao’ ‘Jing qing’ ‘Jing han’ ‘Jing re’ ‘Jing yu’ ‘Jing se’ ‘Jing zhuo’ and ‘Jing hua’ records, establish a male reproductive axis. This paper discusses the relationship between each viscera and semen generation in reproductive axis and gives a syndrome differentiation treatment accroding to the key link of semen production. In this paper,the male reproductive axis is: Tian gui←→Kidney→Nan zi bao→Jing qiao→semen, which is divided into three links: semen source, semen transformation, semen excretion and storage. In the semen source link should strengthen spleen and kidney;in the transformation link should warm yang and nourish blood;in the semen excretion and storage link should soothing liver and nourishing kidney, calming heart and strengthen spleen. According to the pathological factors affecting semen generation, it can supplement deficiency, clear heat, warm Yang, reduce dampness and remove blood stasis. This paper discusses the male reproductive axis comprehensively, which can provide theoretical reference for the clinical treatment of male infertility caused by abnormal semen.
