摘　　要：化疗相关周围神经病变(CIPN)是常见的化疗不良反应之一,以肢体麻木为主要表现,或伴疼痛、遇寒或遇热加重、乏力等兼症。导致CIPN的药物有铂类、紫杉类、长春新碱等,CIPN发生率超过60%,严重影响患者化疗进程及生活质量。李佩文教授认为CIPN基本病机为络脉不通;以疏通络脉为基本治则,认为络脉疏通,则气血畅行、津液输布,麻痛自消;临床根据铂类、紫杉类、长春新碱所致CIPN的不同表现分别采用温经通络、凉血通络、养血通络等法;用药方面善用风药、虫类药以祛邪通络,取得了满意疗效,为临床防治CIPN提供了新思路、新方法。Chemotherapy-induced peripheral neuropathy(CIPN)is one of the most common side effects of chemotherapy,with numbness of the extremities as the main manifestation,or accompanied by pain,cold or heat aggravation,fatigue,and so on.The related chemotherapeutic drugs that lead to CIPN are platinum,taxol,vincristine,etc..More than 60%of patients have CIPN after chemotherapy,severely affecting the treatment of chemotherapy and the quality of life.Professor LI Pei-wen believes the basic pathogenesis of CIPN is the collaterals are blocked.It takes dredging the collaterals as the basic principle of treatment.If the collaterals are dredged,qi and blood can flow freely,body fluid can be transmitted,then the numbness and pain will disappear.According to the different manifestations of CIPN caused by different chemotherapeutic drugs,such as platinum,taxus,vincristine,he adopts the methods of warming yang,cooling blood,nourishing blood to dredge the collaterals.He is expert in medication by using wind and insect medicine to dispel evil and dredge collaterals.He has achieved satisfactory curative effect and then provides a new train of thought and a new method for clinical prevention and treatment of CIPN.
