从益气养阴、补肾活血论治糖尿病肾病

摘　　要：糖尿病肾病是一种常见的继发性肾病,可发展为终末期肾脏病,具有发病率高、发展迅速等特点。何立群教授认为该病病机为本虚标实、虚实夹杂,脾肾亏虚为发病的主要基础,痰瘀内阻为发展的重要因素,内外风邪为加速发展的因素,并且糖尿病肾病的病机是一个动态演变的过程:疾病早期阴虚内热,燥热伤阴;临床期阴伤气耗,气阴两虚;终末期阴损及阳,致阴阳俱虚。治疗以益气养阴、补肾活血为本,重视温阳,善于祛风,多用虫类药搜风通络,并研制出经验效方,临床疗效显著。Diabetic kidney disease(DKD)is a common secondary renal disease that can develop into end-stage renal disease with high incidence and rapid development.Professor HE Li-qun believes that the pathogenesis of the disease are deficiency and excess,the deficiency of spleen and kidney is the root cause of the disease,the internal resistance of phlegm and blood stasis is the important factor of development,and internal and external wind is the factor of accelerating development,and the pathogenesis of DKD is a process of dynamic evolution,including yin deficiency and internal heat are in the early stage of the disease,dryness-heat hurts yin,yin hurts qi in the clinical stage,both qi and yin are deficient,and yin and yang are damaged in the end stage,resulting in yin and yang deficiency.The treatment is based on replenishing qi and nourishing yin,tonifying kidney and promoting blood circulation,paying attention to warming yang,being good at dispelling wind,searching wind and dredging collaterals with insect drugs,and establishing self-made prescription,which has remarkable clinical effect.
