摘 要:目的:运用静息态fMRI的比率低频振幅(fALFF)方法,探索耳穴电刺激引起的正常人自主神经功能改变的中枢调节机制。方法:纳入健康志愿者29例,分别于基线状态、耳穴电刺激及假穴电刺激时行fMRI扫描,采集动态心电图数据,记录心率变异性;对3种不同状态下的心率变异性和fALFF值行单因素重复测量方差分析,并对耳穴电刺激时心率变异性与差异脑区的fALFF值行相关性分析。结果:与基线状态相比,耳穴电刺激RR间期、相邻RR间期差值(MSSD)、相邻RR间期差值大于50 ms的心搏数占总心搏数的百分比(PN50%)均升高,差异均有统计学意义(均P<0.05);假穴电刺激与基线状态的RR间期、MSSD、PN50%比较,差异均无统计学意义(均P>0.05)。耳穴电刺激时边缘叶、海马旁回及颞叶局部脑区的fALFF值升高(P<0.05,FWE矫正),且差异脑区的fALFF值与低频功率、高频功率两者的比值(LF/HF值)呈正相关(r=0.422,P=0.023)。结论:耳穴电刺激可调节正常人自主神经功能,同时引起边缘叶、海马旁回及颞叶的fALFF改变,且两者间具有相关性,这为研究耳穴电刺激疗法的机制提供了新的神经影像学基础及生理学证据。Objective:To explore the central mechanism of autonomic nerve function changes after auricular electrical stimulation in normal population by resting-state fMRI with fractional amplitude of low-frequency fluctuations(fALFF).Methods:Twenty-nine healthy volunteers were enrolled and underwent fMRI at 3 states,the baseline state,electrical stimulation on auricular true point and sham point.One-way analysis of variance was used to compare heart rate variability and fALFF value in 3 different states,and correlation between fALFF values of different brain regions and heart rate variability during auricular true stimulation was analyzed.Results:Compared with the baseline state,there was a statistically significant increase in the RR interval,MSSD,and PN50%in true stimulation and no significant difference in sham stimulation.During the true point electrical stimulation,the fALFF values increased in the limbic lobe,parahippocampal gyrus and temporal lobe regions(P<0.05,FWE correct),and their fALFF values showed a positive correlation with ratio of low frequency to high frequency(LF/HF ratio)(r=0.422,P=0.023).Conclusions:Auricular point electrical stimulation can regulate autonomic function of normal human,changes fALFF values in the limbic lobe,hippocampal gyrus and temporal lobe,and there is a correlation between each other,which provides a new neuroimaging basis and physiologic evidence for studying the mechanisms of auricular point electrical stimulation.
