本地6例鹦鹉热肺炎的影像学表现及临床特征

摘　　要：目的:探讨本地偶发性6例鹦鹉热肺炎患者的影像表现和临床特征。方法:回顾性分析6例鹦鹉热肺炎患者的临床资料、X线及CT资料。结果:6例中3例为一家三口聚集性发病,另外3例为个体发病。其中1例伴多种基础性疾病,治疗无效后死亡。6例影像表现相似,但由于进展期不同略有差异。X线胸片表现为不同程度的大片肺实变影,边界模糊,形成白肺。CT表现为非典型肺炎征象。结论:鹦鹉热肺炎的临床及影像表现虽特异性较差,但有一定特点,结合接触史、实验室检查可确诊;采用二代宏基因测序(mNGS)技术进行快速诊断也是鉴别鹦鹉热肺炎的重要方法。Objective:To investigate the imaging manifestations and clinical features of 6 patients of local sporadic psittacosis pneumonia.Methods:The clinical data,X-ray and CT images data of 6 patients were retrospectively analyzed.Results:Among the 6 patients,3 were in a family cluster,and the other 3 were sporadic.One patient had multiple underlying diseases and died after failing to treatment.The imaging manifestations of 6 cases were similar but slightly different due to different stages of the disease.The chest X-ray showed that all of them had varying degrees of large lung consolidation shadows with blurred boundaries,formed‘white lung’.CT images showed signs of atypical pneumonia.Conclusions:The characteristics of psittacosis pneumonia is the lack of specificity in clinical and imaging findings,but it can be confirmed with the combination of exposure history and laboratory tests Although the clinical and imaging specificity of psittacosis pneumonia is poor,it has certain characteristics.Combining the contact history and laboratory tests,the diagnosis can be confirmed,and metagenomic next-generation sequencing(mNGS)technology for rapid diagnosis is also an important method for diagnosing psittacosis pneumonia.
