摘 要:目的:评估美国放射学会(ACR)发表的卵巢附件报告和数据系统(O-RADS)分类对附件病变良恶性诊断的有效性。方法:前瞻性收集经手术病理证实的附件病变患者80例(82个病变),均行阴道和/或经腹超声检查。通过O-RADS分类对病变进行风险分类,并绘制ROC曲线,评价O-RADS分类对附件病变良恶性诊断的有效性。结果:82个病变中,病理为良性47个,恶性35个。O-RADS分类ROC曲线的AUC为0.923(95%CI 0.866~0.981),O-RADS 3类为最佳临界点。O-RADS分类诊断附件病变良恶性的敏感度为97.1%,特异度为66.7%。结论:O-RADS分类对附件病变良恶性的诊断有效性良好。Objective:To evaluate the effectiveness of the ovarian-adnexal reporting and data system(O-RADS)published by American College of Radiology(ACR)in the diagnosis of benign and malignant gynecological adnexal lesions.Methods:A total of 82 adnexal lesions of 80 patients confirmed by operation and pathology were prospectively collected.All patients underwent transvaginal and/or transabdominal ultrasonography.The lesions were graded according to O-RADS,and the ROC curve was obtained,and the validity of O-RADS classification in the diagnosis of benign and malignant adnexal lesions was evaluated.Results:Among the 82 adnexal lesions,47 were benign and 35 were malignant conformed by pathology.The AUC of ROC curve of the O-RADS classification was 0.923(95%CI 0.866~0.981).O-RADS Category 3 was the best cut-off value,the sensitivity of O-RADS diagnosis was 97.1%,and the specificity was 66.7%.Conclusion:O-RADS category is effective in the diagnosis of benign and malignant adnexal lesions.
