马德龙病合并喉癌1例

摘　　要：马德龙病是一种病因不明的脂肪代谢障碍疾病。本文报道1例马德龙病合并喉癌患者。该患者临床表现为进行性声嘶及呼吸、吞咽困难,其颈部、颏下、颌下等有弥漫性对称性肿胀。喉动态镜检查:咽喉部巨大菜花样新生物,左侧声带固定。喉部CT:喉癌(贯声门型),左侧颈静脉链区淋巴结转移征象,颈部Madelung综合征。生化检查:白蛋白38.7 g/L,球蛋白27.5 g/L,前白蛋白160 g/L,谷草转氨酶14 IU/L,γ-转肽酶80 IU/L,谷丙转氨酶7 IU/L,肌酐43μmol/L。术前病理回报:鳞状细胞癌。入院诊断:喉癌(贯声门型T4N1M0);Ⅲ度喉梗阻;马德龙病;脂肪肝。该患者经手术治疗后恢复良好。Madelung’s disease is a lipodystrophy of unknown etiology.This article reports a case of Madelung’s disease complicated with laryngeal cancer.The clinical manifestations of the patient were progressive hoarseness and dyspnea,dysphagia,and diffuse symmetrical swelling of the neck,submental,and submandibular.Dynamic laryngoscopy revealed a giant cauliflower-like neoplasm in the throat,with the left vocal cord fixed.Laryngeal CT showed laryngeal carcinoma(transglottic type),signs of lymph node metastasis in the left jugular chain region,and Madelung syndrome in the neck.Biochemical tests showed albumin 38.7 g/L,globulin 27.5 g/L,prealbumin 160 g/L,aspartate aminotransferase 14 IU/L,γ-transpeptidase 80 IU/L,alanine aminotransferase 7 IU/L,Creatinine 43μmol/L.Preoperative pathology suggested squamous cell carcinoma.Admission diagnosis included laryngeal cancer(transglottic T4 N1 M0),Ⅲdegree laryngeal obstruction,Madelung’s disease and fatty liver.The patient recovered well after surgery.
