摘  要:目的:系统评价Teach-back健康教育对脑卒中病人康复效果的影响。方法:计算机检索PubMed、EMbase、Web of Science、the Cochrane Library、CINAHL、中国生物医学文献数据库(SinoMed)、中国知网、维普数据库、万方数据库。检索时间均为建库至2022年1月。依据Cochrane手册6.1.0质量评价标准对纳入文献进行评价,采用RevMan 5.4软件进行系统评价。结果:共纳入11项研究,共1 018例病人。Meta分析结果显示,试验组肢体运动功能[MD=9.98,95%CI(7.19,12.77),P<0.000 01]、焦虑情绪[SMD=-0.86,95%CI(-1.31,-0.42),P=0.000 1]、抑郁情绪[SMD=-0.99,95%CI(-1.41,-0.57),P<0.000 01]、健康素养[SMD=2.66,95%CI(2.26,3.07),P<0.000 01]、日常生活自理能力[SMD=1.52,95%CI(0.83,2.21),P<0.000 1]改善程度均优于对照组,差异有统计学意义。两组自我效能比较,差异无统计学意义[MD=7.08,95%CI(-4.42,18.57),P=0.23]。结论:Teach-back健康教育可有效改善脑卒中病人的肢体运动功能,缓解焦虑和抑郁情绪,提高健康素养及日常生活自理能力,但尚不能证实对自我效能的影响。此外,由于纳入文献质量不高,研究结论仍需要更多大样本、多中心的高质量随机对照试验进一步验证。Objective:To systematically evaluate the rehabilitation effect of Teach-back health education on stroke patients.Methods:PubMed, EMbase, Web of Science, the Cochrane Library, CINAHL,SinoMed, CNKI,VIP,WanFang Database were searched.The retrieval time was from the establishment of the database to January 2022.The included articles were evaluated according to the quality evaluation criteria of the Cochrane Manual 6.1.0,and the RevMan 5.4 software was used for systematic evaluation.Results:A total of 11 studies with a total of 1 018 patients were included.The results showed that motor function[MD=9.98,95%CI(7.19,12.77),P<0.000 01],anxiety[SMD=-0.86,95%CI(-1.31,-0.42),P=0.000 1],depression[SMD=-0.99,95%CI(-1.41,-0.57),P<0.000 01],health literacy[SMD=2.66,95%CI(2.26,3.07),P<0.000 01],self-care ability in daily life[SMD=1.52,95%CI(0.83,2.21),P<0.000 1]were better than the control group, and the difference was statistically significant.There was no significant difference in self-efficacy[MD=7.08,95%CI(-4.42,18.57),P=0.23]between the two groups.Conclusions:Teach-back health education can effectively improve the physical motor function of stroke patients, relieve anxiety and depression, improve health literacy and self-care ability in daily life, but the effect on self-efficacy can not be confirmed.In addition, due to the low quality of the included literature, the conclusions of this study still need to be further verified by more high-quality randomized controlled trials with large samples and multi-centers.
