机械通气患者早期康复护理质量改进模式的应用效果

摘　　要：目的探讨质量改进模式在机械通气患者早期康复中的应用。方法将2020年1—12月ICU收治的机械通气患者62例设为对照组,行常规性护理管理;将2021年1—12月ICU收治的机械通气患者65例设为观察组,行质量改进模式。比较两组患者并发症发生情况、预后情况及护理服务满意程度。结果行质量改进模式干预后,观察组ICU获得性衰弱(ICU-AW)、谵妄、呼吸机相关性肺炎(VAP)发生率均低于对照组(P<0.05)。两组患者气管切开、拔管失败发生率经比较差异无统计学意义(P>0.05)。观察组谵妄持续时间、机械通气时间、入住ICU时间及总住院时间短于对照组(P<0.05)。观察组对机械通气护理满意程度高于对照组(P<0.05)。结论质量改进模式能有效降低机械通气患者并发症发生率,缩短患者机械通气时间及入住ICU时间,有利于患者康复,提高患者护理满意程度。Objective To explore the application of quality improvement model in early rehabilitation of patients with mechanical ventilation was as the study subject.Methods A total of 62 patients with mechanical ventilation admitted to ICU from January to December 2020 were set as a control group,and routine nursing management was carried out;65 cases of mechanical ventilation patients admitted to ICU from January to December 2021were set as an observation group,and the quality improvement mode was adopted.The complications,prognosis and satisfaction of nursing service were compared between the two groups.Results After the intervention of quality improvement mode,the incidence of ICU-acquired weakness(ICU-AW),delirium and ventilator associated pneumonia(VAP)in the observation group was lower than thatin the control group(P<0.05).There was no significant difference in the incidence of tracheotomy and extubation failure between the two groups(P>0.05).The duration of delirium,time of mechanical ventilation,stay in ICU and total hospital stay in the observation group were shorter than those in the control group(P<0.05).The nursing satisfaction of mechanical ventilation in the observation group was higher than that in the control group(P<0.05).Conclusion The quality improvement model can effectively reduce the incidence of complications in patients with mechanical ventilation,shorten the time of mechanical ventilation and stay in ICU,help patients recover,and improve patient nursing satisfaction.
