空心拉力钉张力带和克氏针张力带治疗髌骨横型骨折疗效研究

摘　　要：目的 比较髌骨横型骨折患者采用空心拉力钉张力带、克氏针张力带治疗的临床效果。方法 选择2019年6月~2021年12月皖南医学院第一附属医院骨科住院接受髌骨横型骨折切开复位内固定治疗的患者30例,采用随机数字表法将其分成两组,每组15例。对照组采用克氏针张力带内固定治疗,观察组给予空心拉力钉张力带内固定,比较两组手术时间、术中失血量、住院时间、骨折愈合时间;随访6个月,比较两组术后膝关节视觉模拟评分(VAS)、Lysholm评分,并比较两组疗效优良率。结果 两组手术时间、术中失血量比较,差异无统计学意义(P>0.05)。观察组住院时间、骨折愈合时间均较对照组短,差异有统计学意义(P<0.05)。观察组术后6个月时膝关节VAS评分较对照组低,Lysholm评分较对照组高,观察组治疗优良率100.00%,对照组60.00%高,二组比较差异有统计学意义(P<0.05)。结论 髌骨横型骨折患者采用空心拉力钉张力带内固定治疗,效果较克氏针张力带好,膝关节疼痛更轻且功能改善更理想,并能缩短住院时间,加速康复进程,应用价值较好。Objective To compare the therapeutic effect of tension band of hollow tension nail and Kirschner wire on transverse fracture of patella.Methods From June 2019 to December 2021, 30 patients with patellar fracture treated in Department of Orthopedics, the First Affiliated Hospital of Wannan Medical College were retrospectively studied. They were randomly divided into two groups, each group had 15 cases. The control group was treated with Kirschner wire tension band internal fixation, and the observation group was treated with hollow tension nail tension band internal fixation.Operation time, intraoperative blood loss, hospital stay and fracture healing time were recorded and compared between the two groups.The two groups were followed up for 6 months. Visual analogue scale(VAS) score and Lysholm score were compared between the two groups, and the good efficacy rate of the two groups was evaluated.Results There were no significant differences in operative time and intraoperative blood loss between the two groups(P>0.05).The hospitalization time and fracture healing time of observation group were shorter than those of control group(P<0.05).The knee VAS score of the observation group 6 months after surgery was lower than that of the control group(P<0.05).Lysholm score was higher than that of control group(P<0.05).The rate of excellent and good treatment in observation group(100.00%) was higher than that in control group(60.00%)(P<0.05).Conclusion For patients with patella transverse fracture, hollow tension nail tension band internal fixation has better effect than Kirschner wire tension band, and the knee joint pain is lighter and the function is better. It can shorten the hospital stay and accelerate the rehabilitation process, and has outstanding application value.
