眼科50岁及以上住院手术患者疾病谱构成帕累托图分析

摘　　要：目的 分析某眼科医院50岁及以上住院手术患者眼部疾病构成情况,为眼科医疗保障及服务能力建设提供参考依据。方法 采用回顾性研究,收集某院2016年1月1日-2020年12月31日眼科50岁及以上手术患者住院病案首页信息,依据ICD-10对主要诊断进行分类,运用帕累托图分析疾病构成情况。结果 50岁及以上手术患者人数除2020年外呈上升趋势,各年间的疾病谱顺位一致(WR=0.9367,P<0.05)。排名前10的疾病分别是白内障占61.1%、视网膜脉络膜疾病占7.1%、青光眼占6.8%、黄斑疾病占4.8%、硅油眼占3.8%、结膜疾病占2.8%、糖尿病性眼病占2.8%、泪器疾病占1.4%、角膜疾病占1.4%和眼肿瘤占1.3%,共占93.3%。结论 5年间,某眼科医院白内障占比逐年下降,眼底疾病、青光眼占比逐年上升,加强白内障、眼底疾病手术防治工作以及临床医疗管理,提升临床医疗服务效率是当前的工作重点。Objectives To analyze eye disease structures and syn-position situation in patient aged 50 and above in recent 5 years, and provide reference for the construction of medical security and service capacity of ophthalmic inpatients over 50 years old. Methods A retrospective study was conducted to collect and sort the front page information of ophthalmology patients who 50 and above from January 1st, 2016 to December 31st, 2020. The international classification of diseases(ICD) 10 was used to classify the principal diagnosis, and the pareto chart analysis were used to analyze the disease composition. Results In recent 5 years, the number of surgical patients aged 50 and above showed an increased trend except 2020.The syn-position of disease was consistent in each year(WR=0.9367,P<0.05). The top 10 diseases included cataract(61.1%), retina and choroid disease(7.1%), glaucoma(6.8%), maculopathy(4.8%), silicone eye(3.8%), conjunctiva disease(2.8%), diabetic eye disease(2.8%), lacrimal apparatus disease(1.4%), corneal disease(1.4%) and tumor(1.3%), accounted for 93.3%. Conclusions The proportion of cataract decreased during study period, with fundus diseases(retina and choroid disease, maculopathy, silicone oil eye, diabetic eye disease) and glaucoma increased in recent 5 years.The work point in the near future should be focused on the prevention and treatment of cataract and fundus oculi diseases. In the meantime, it was important to strengthen clinical medical management and improve the efficiency of clinical medical service.
