中医保守治疗颈椎间盘突出症住院费用的影响因素分析

摘　　要：目的 探讨中医保守治疗颈椎间盘突出症患者住院费用的影响因素,为患者减轻患者经济负担提供参考依据。方法 选择某三甲中医医院2018年1月1日-2019年12月31日明确主要诊断为颈椎间盘突出症的患者545例住院病案信息,采用统计学描述性分析、非参数检验、多元线性回归方程对住院费用影响因素进行综合分析。结果 西药费、针灸推拿治疗费、检查检验费占比分别为35.16%、21.26%和13.26%,为颈椎间盘突出症住院费用中的前3位;经统计分析,F=5.783,P<0.05,差异具有统计学意义,R^(2)=0.282。不同的性别、年龄、住院天数、合并症、疾病程度的颈椎间盘突出症患者,住院费用间的差异具有统计学意义,P<0.05;不同参保类型、证型、突出层面的颈椎间盘突出症患者住院费用比较,差异无统计学意义,P>0.05。结论 颈椎间盘突出症患者住院费用影响因素重要性排序依次是合并症、性别、参保类型、年龄、疾病程度、住院天数。通过缩短住院天数、减少合并症等措施,合理降低医疗成本,以达到降低住院费用,减轻患者及其家庭的经济负担的目的。Objectives To explore the factors influencing the hospitalization expenses of patients with cervical disc herniation treated conservatively in traditional Chinese Medicine, and provide reference data for reducing the economic burden of patients. Methods From January 1st, 2018 to December 31st, 2019, 545 patients diagnosed with cervical disc herniation in a Three A and Tertiary Hospital of Traditional Chinese Medicine in Heilongjiang Province were selected, and the influencing factors of hospitalization expenses were comprehensively analyzed by statistical descriptive analysis, non-parametric test and multiple linear regression equation. Results Western medicine fees,(acupuncture and massage) treatment fees, and inspection fees accounted for 35.16%, 21.26%, and 13.26%,respectively, which were the top three in hospitalization expenses for cervical disc herniation;F=5.783, P<0.05,according to statistics. Statistically significant, R^(2)=0.282. In patients with cervical disc herniation with different gender, age, hospitalization stay length, comorbidities, and degree of disease, the difference in hospitalization expenses was statistically significant(P<0.05). There was no statistically significant difference in hospitalization expenses among patients with schizophrenia(P>0.05). Conclusions The order of importance of influencing factors of hospitalization expenses in patients with cervical disc herniation was comorbidity, gender, type of insurance, age,degree of disease, and days of hospitalization. Through measures such as shortening the length of hospital stay and controlling the comorbidities of patients, the medical cost could be reasonably reduced, so as to reduce the hospitalization expenses and reduce the economic burden of the patients and their families.
