祛腐生肌散加自拟清热解毒汤用于泛发性肛周脓肿术后疗效观察

摘　　要：目的 探讨祛腐生肌散外敷联合自拟清热解毒汤用于泛发性肛周脓肿术后对创腔愈合和肛门功能的影响。方法 选取2018年1月—2020年12月收治的150例泛发性肛周脓肿患者为研究对象,按照随机数表法分为研究组与对照组各75例。对照组采取常规治疗,研究组予以祛腐生肌散外敷联合自拟清热解毒汤治疗。分析治疗2周后,两组患者中医疗效差异;比较两组患者在治疗前及治疗2周后,其中医证候(肛周红肿、触痛明显、表面焮热)积分变化;记录两组患者术后首次排便时间及创腔愈合时间差异;比较治疗前、治疗1周后及治疗2周后,两组患者肛门功能(Wexner量表)评分变化;记录两组患者在术后2周内并发症发生情况差异。结果 研究组患者在治疗2周后的总有效率93.33%,较对照组的81.33%高(P<0.05);治疗2周后,研究组肛周红肿、触痛明显、表面焮热各中医证候积分分别为(1.11±0.72)分、(1.35±0.74)分、(1.12±0.56)分,低于对照组的(1.81±0.75)分、(2.12±0.66)分、(1.61±0.53)分(P<0.05);研究组术后首次排便时间(41.85±6.53)h及创腔愈合时间(21.71±3.90)d短于对照组的(53.67±6.95)h、(26.96±3.93)d(P<0.05);治疗1周、2周后,研究组Wexner评分(5.20±1.23)分、(2.65±0.44)分,低于对照组同时间点的(7.58±1.25)分、(5.60±0.98)分(P<0.05);术后2周内,研究组并发症总发生率4.00%,较对照组的14.67%低(P<0.05)。结论 祛腐生肌散外敷联合自拟清热解毒汤有助于改善泛发性肛周脓肿患者疾病症状与临床疗效,强化其肛门功能,并减少术后并发症。Objective To explore the effects of external application of Qufu Shengji Powder(祛腐生肌散)combined with Self-made Qingre Jiedu Decoction(自拟清热解毒汤)on wound cavity healing and anal function after generalized perianal abscess surgery.Methods A total of 150 patients with generalized perianal abscess treated between January 2018 and December 2020 were selected as the research subjects,and they were divided into study group and control group according to the random number table method,with 75 cases in each group.The control group was given conventional treatment,and the study group was treated with external application of Qufu Shengji Powder combined with Self-made Qingre Jiedu Decoction.The difference in TCM efficacy was analyzed in the two groups after 2 weeks of treatment.The changes in scores of TCM symptoms(perianal redness and swelling,obvious haphalgesia,superficial heat)were compared between the two groups before treatment and after 2 weeks of treatment.The postoperative first defecation time and wound cavity healing time were recorded in the two groups.The score of anal function(Wexner scale)of the two groups was compared before treatment,after 1 and 2 weeks of treatment.The occurrence of complications within 2 weeks after surgery were recorded in the two groups.Results The total effective rate was 93.33%in the study group after 2 weeks of treatment,which was higher than 81.33%in control group(P<0.05).After 2 weeks of treatment,the TCM syndromes scores of perianal redness and swelling,obvious haphalgesia and superficial heat were(1.11±0.72)points,(1.35±0.74)points and(1.12±0.56)points in the study group,which were lower than(1.81±0.75)points,(2.12±0.66)points and(1.61±0.53)points in the control group(P<0.05).The postoperative first defecation time and wound cavity healing time were(41.85±6.53)h and(21.71±3.90)d in the study group,which were shorter than(53.67±6.95)h and(26.96±3.93)d in the control group(P<0.05).After 1 and after 2 weeks of treatment,the Wexner scores were(5.20±1.2
