活血化瘀中药穴位贴敷对药物流产成功率影响的临床观察

摘　　要：目的 探讨活血化瘀中药穴位贴敷干预对药物流产成功率的影响。方法 将90例自愿要求终止妊娠的早孕健康孕妇随机分成西药组和联合组,两组各45例。西药组给予米非司酮片和米索前列醇片进行药物流产,联合组在西药组基础上,另给予活血化瘀中药(红花、川芎、当归制成药膏)穴位贴敷(次髎、三阴交)干预。记录并比较两组药物流产成功率、孕囊排出时间、阴道流血状况以及不良反应情况。结果 联合组药物流产成功率95.56%(43/45)高于西药组80.00%(36/45),孕囊排出时间、阴道流血持续时间均短于西药组,阴道出血量多于月经量的比重11.11%(5/45)低于西药组31.11%(14/45),差异均有统计学意义(P<0.05),两组月经复潮时间比较,差异无统计学意义(P>0.05),均未出现明显不良反应。结论 活血化瘀中药穴位贴敷干预能促进孕囊排出,提高药物流产的成功率,改善阴道出血状况,疗效和安全性显著。Objective To explore the effect of acupoint application of traditional Chinese medicine for promoting blood circulation and removing blood stasis on the success rate of drug abortion.Methods Ninety healthy pregnant women who voluntarily requested termination of pregnancy were randomly divided into western medicine group and combined group, with 45 cases in each group.The western medicine group was given mifepristone tablets and misoprostol tablets for drug abortion.On the basis of the western medicine group, the combined group was given traditional Chinese medicine for promoting blood circulation and removing blood stasis[ointment made of Honghua(Carthami Flos),Chuanxiong(Chuanxiong Rhizoma)and Danggui(Angelicae Sinensis Radix)] and acupoint application[Ciliao(BL32)and Sanyinjiao(SP6)].The success rate of drug abortion, the discharge time of gestational sac, vaginal bleeding and adverse reactions were recorded and compared between the two groups.Results The success rate of drug abortion in the combined group was 95.56%,higher than 80.00% in the western medicine group.The discharge time of gestational sac and the duration of vaginal bleeding were shorter than those in the western medicine group.The proportion of vaginal bleeding more than menstrual volume was 11.11%,lower than 31.11% in the western medicine group.The difference was statistically significant(P<0.05).There was no significant difference in monthly menstrual rehydration time between the two groups(P>0.05).There were no obvious side effects.Conclusion Acupoint application of traditional Chinese medicine for promoting blood circulation and removing blood stasis can promote the discharge of gestational sac, improve the success rate of drug abortion and improve the condition of vaginal bleeding.It has significant curative effect and safety.
