摘  要:目的探究晚孕期胎儿脾动脉(SPA)血流参数联合脐血流(UA)在预测胎儿宫内缺氧中的价值。方法选取2020年1月—2022年2月在焦作市妇幼保健院行超声检查的114例孕31周~40^(+6)周单胎胎儿作为观察对象,按照检查结果分为正常对照组81例及异常观察组33例,并将其按孕周分别分为31~33^(+6)周,34~35^(+6)周,36~40^(+6)周三组,检测观察组和对照组患者胎儿SPA和UA的阻力指数(RI)、搏动指数(PI)、收缩期峰值流速/舒张期峰值流速(S/D)值,分析SPA与UA的RI比值、S/D比值、PI比值的在评估胎儿宫内缺氧中价值。结果观察组与对照组对比分析,观察组SPA与UA的S/D比值均低于对照组,具有统计学意义(P<0.05);计算其敏感性特异性:当两者联合检测时,对胎儿宫内缺氧的诊断具有较高的敏感性与特异性。结论联合检测脾动脉与脐血流对早期发现胎儿宫内缺氧具有重要的价值,是一种简单方便、有效实用、无创的补充方法。Objective To explore the value of fetal splenic artery(SPA)flow parameters combined with umbilical blood flow(UA)in predicting fetal intrauterine hypoxia during late pregnancy.Methods Totally 114 singleton fetuses(31 W-40^(+6)W)who had tests in Jiaozuo maternal and child health hospital from January 2020 to february 2022 were selected and divided into normal control group(n=81)and abnormal observation group(n=33).They were divided into three groups according to gestational weeks:31-33^(+6)W,34-35^(+6)W,36-40^(+6)W.The resistance index(RI),pulsatility index(PI),systolic peak flow velocity compared to diastolic peak flow velocity(S/D)of Spa and UA were measured,then the value of RI,S/D and PI of SPA and UA in evaluating fetal hypoxia were analyzed.Results The S/D ratio of SPA and UA in the observation group was significantly lower than that of the control group(P<0.05).It has high sensitivity and specificity in the diagnosis of fetal intranterine hypoxia when combined the two methods.Conclusion The combined detection of splenic artery and umbilical blood flow is of great value in early detection of fetal hypoxia,and is a simple,effective,non-invasive complementary method.
