辨证论治肛门直肠神经官能症疗效观察

摘　　要：目的:观察中医辨证论治肛门直肠神经官能症的临床疗效。方法:将120例肛门直肠神经官能症患者随机分为对照组和观察组,每组60例。对照组采用氟哌噻吨美利曲辛片口服治疗,观察组予以中医辨证论治口服中药治疗,两组治疗周期均为4周。观察治疗后两组临床疗效、抑郁评分、焦虑评分、疼痛评分及中医证候评分变化。结果:治疗后,观察组总有效率[86.60%(52/60)]显著高于对照组[70.00%(42/60)](P<0.05)。治疗后两组抑郁评分、焦虑评分、疼痛评分及中医证候积分均明显低于治疗前(P<0.05);观察组治疗后抑郁评分、焦虑评分与对照组比较,差异无统计学意义(P>0.05);观察组治疗后疼痛评分及中医证候积分均低于对照组(P<0.05)。结论:中医辨证论治肛门直肠神经官能症,较单独使用抗抑郁、焦虑药物临床疗效更加显著,除可缓解患者焦虑、抑郁状态外,更有利于减轻肛门神经疼痛、坠胀、灼热等异常感觉。Objective:To observe clinical effects of syndrome differentiation and treatment of anorectal neurosis.Methods:All 120 patients were randomized into the control group and the observation group,60 cases in each group.The control group was treated by Flupentixol and Melitracen tablets orally,and the observation group by syndrome differentiation and treatment and oral administration of herbs,both groups were treated for four weeks.To observe clinical effects,SAS,SDS,VAS and TCM syndrome integrals of the two groups after the treatment.Results:After the treatment,total effective rate of the observation group was[86.60%(52/60)],significantly higher than[70.00%(42/60)]of the control group(P<0.05).After the treatment,SDS,SAS,VAS and TCM syndrome integrals of both groups were lower than these before the treatment distinctly(P<0.05);the difference had no statistical meaning when SAS and SDS after the treatment of the observation group were compared with these of the control group(P>0.05);the observation group was lower than the control group in VAS and TCM syndrome integrals(P<0.05).Conclusion:Syndrome differentiation and treatment of anorectal neurosis are superior to simple use of antidepressant drug and anxiolytics evidently,it could relieve anxiety and depression state,and alleviate the abnormal feelings such as anal nerve pain,bearing down and distension,and scorching hot.
