摘 要:目的观察不同糖皮质激素治疗成人突发性耳聋对睡眠的影响。方法回顾性选择2017年12月—2019年9月东南大学医学院附属南京同仁医院收治的突发性耳聋患者201例,根据治疗方法不同分为对照组(n=32)、地塞米松组(n=46)及甲泼尼龙组(n=123),对照组未应用糖皮质激素治疗,地塞米松组应用地塞米松治疗,甲泼尼龙组应用甲泼尼龙琥珀酸钠治疗,比较3组患者匹兹堡睡眠质量指数量表(PSQI)评分。结果治疗第3天,对照组患者主观睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能障碍评分及PSQI总分与治疗前比较差异均无统计学意义(P>0.05);治疗第3天,地塞米松组与甲泼尼龙组患者主观睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物及PSQI总分均较治疗前升高(P<0.05),但甲泼尼龙组患者变化幅度小于地塞米松组(P<0.01)。地塞米松组患者停用激素第3天与停用激素第1天主观睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能障碍评分及PSQI总分比较差异均无统计学意义(P>0.05);甲泼尼龙组患者停用激素第3天主观睡眠质量、入睡时间、PSQI总分均低于停用激素第1天(P<0.05)。结论应用糖皮质激素治疗对突发性耳聋患者的睡眠状况有影响,地塞米松对患者睡眠的影响大于甲泼尼龙琥珀酸钠,停用后,地塞米松后期对睡眠的影响时间较甲泼尼龙琥珀酸钠长。Objective To observe the effect of different glucocorticoids on sleep in adults with sudden deafness.Methods A total of 201 patients with sudden deafness admitted to Nanjing Tongren Hospital Affiliated to Southeast University School of Medicine from December 2017 to September 2019 were retrospectively selected and divided into control group(n=32),dexamethasone group(n=46)and methylprednisolone group(n=123)according to different treatment methods.The control group was not treated with glucocorticoids,the dexamethasone group was treated with dexamethasone,and the methylpred nisolone group was treated with methylprednisolone sodium succinate.The Pittsburgh Sleep Quality Index(PSQI)scores were compared among the three groups.Results On the third day of treatment,there were no significant differences in subjective sleep quality,falling asleep time,sleep time,sleep efficiency,sleep disorders,hypnotic drugs,daytime dysfunction score and total PSQI score compared with those before treatment in the control group(P>0.05).On the third day of treatment,the subjective sleep quality,falling asleep time,sleep time,sleep efficiency,sleep disorders,hypnotic drugs and PSQI total score of patients in the dexamethasone group and the methylprednisolone group were significantly higher than those before treatment(P<0.05),but the range of changes in the methylprednisolone group were smaller than falling asleep time in the dexamethasone group(P<0.01).In the dexamethasone group,there were no significant differences in subjective sleep quality,falling asleep time,sleep time,sleep efficiency,sleep disorders,hypnotic drugs,daytime dysfunction score and total PSQI score on the third day and the first day of hormone withdrawal(P>0.05);In the methylprednisolone group,the subjective sleep quality,sleep time,and PSQI total score on the third day were lower than those on the first day(P<0.05).Conclusion Glucocorticoid treatment has an effect on the sleep status of patients with sudden deafness,the effect of dexamethasone on sleep is greater than t
