摘　　要：目的对当前典型手指逆行岛状皮瓣修复术的最新文献进行总结,分析手部供区修复存在的问题,通过手术设计为手部供区修复问题寻找更优的解决方案。方法本文系统检索了4个国内外医学研究数据库中关于局部皮瓣修复手部供区的文献,对符合文献筛选标准的2018至2022年发表的67篇文献的结果进行了系统分析,并对纳入文献进行梳理、归纳、总结,结合具体病例分析目前常用的3种供区修复方法的优缺点。通过设计手背局部皮瓣的方案,探索其对供区修复的可行性及先进性。结果通过对文献与病例进行系统性分析,发现局部皮瓣在设计方案中均可对手指近端供区进行修复,同时可解决目前手部供区修复中存在的张力过大、瘢痕、植皮收缩、皮片色差等问题。结论采用局部皮瓣不仅可对手指近端供区缺损进行良好修复,而且手术操作相对简单,是手部供区修复的更优选择。Objective To summarize the latest literature on the current typical finger retrograde island flap repair,analyze the problems existing in the repair of the hand donor site,and find a better solution for the problem of hand donor site repair through surgical design.Methods Four domestic and foreign medical research databases were retrieved for the literature on local flap repair of the hand donor area.Furthermore,we systematically analyzed the results of 67 studies published from 2018 to 2022 that met the literature screening criteria.Moreover,the included literature was sorted out,summarized,and combined with specific cases,the advantages and disadvantages of the three commonly-used operations were discussed.Finally,we shed new sights on the feasibility and advancement of the restoration methods for the donor areas.Results Through the systematic analysis of the literature and cases,it was found that the local flap could repair the donor area of proximal finger in the design scheme,and solve the existing problems including excessive tension,surgical scar,skin graft contraction and color difference.Conclusion The local flap can not only repair the defect of the donor area in the proximal finger,but also be a better choice for repairing the donor area of the hand with a relatively simple operation.
