摘  要:目的探讨磁共振成像(magnetic resonance imaging,MRI)在髌下脂肪垫撞击综合征(又称Hoffa病)中的成像特点及应用价值。方法通过回顾性分析2019年1月至2021年1月在天津市东丽医院经临床诊断及随访确诊为髌下脂肪垫损伤的Hoffa病患者125例,分析其MRI特征性表现。结果125例中,右膝62例,左膝51例,双膝12例,共137个膝关节出现不同程度的髌下脂肪垫损伤,其中Ⅰ级62个(45.3%),Ⅱ级41个(29.9%),Ⅲ级25个(18.2%),Ⅳ级9个(6.6%)。依据Hoffa病的髌下脂肪垫损伤的影像分级来分别指导临床给予相应治疗。终止治疗1~2个月后进行随访。采用视觉模拟评分法(visual analogue scale,VAS)作为观测指标,比较治疗前后的差异。四组患者治疗前后VAS评分差异均有统计学意义(P<0.05)。结论MRI能清晰显示Hoffa病髌下脂肪垫损伤的形态及信号改变,并能准确评估其损伤的轻重程度及伴随病变,可为临床治疗提供相应的影像学依据。Objective To explore the imaging characteristics and application value of magnetic resonance imaging(MRI)in subpatellar fat pad impingement syndrome(or Hoffa’s disease).Methods A total of 125 patients with Hoffa’s disease diagnosed as infrapatellar fat pad injury in Tianjin Dongli Hospital from January 2019 to January 2021 were retrospectively analyzed,and their MRI features were analyzed.Results Among the 125 cases,there were 62 cases of right knee,51 cases of left knee and 12 cases of both knees.A total of 137 knees showed subpatellar fat pad edema of varying degrees,including 62 cases of gradeⅠ(45.3%),41 cases of gradeⅡ(29.9%),25 cases of gradeⅢ(18.2%)and 9 cases of gradeⅣ(6.6%).The imaging grading of Hoffa’s subpatellar fat pad injury guided the clinical treatment.Follow-up was performed 1~2 months after termination of treatment.Visual analogue scale(VAS)was used as the observation index to compare before and after treatment.VAS scores of 4 groups before and after treatment were statistically significant(P<0.05).Conclusion MRI can clearly show the morphology and signal changes of Hoffa’s subpatellar fat pad injury,and accurately assess the severity of the injury,which can provide corresponding imaging basis for clinical treatment.
