血府逐瘀汤结合紫杉醇+奈达铂方案对气滞血瘀证宫颈癌患者化疗毒副作用及生存期的影响

摘　　要：目的 探讨血府逐瘀汤结合标准TP方案(紫杉醇+奈达铂)对气滞血瘀证宫颈癌患者化疗毒副作用和生存期的影响。方法 选取2015年12月到2019年11月在安徽省天长市中医院接受治疗的90例气滞血瘀证宫颈癌患者作为研究对象,按照随机对照原则进行1∶1分组,为对照组和研究组,各45例。所有患者均给予标准TP方案治疗,研究组同时给予血府逐瘀汤治疗,比较两组化疗效果、毒副作用及生存质量。结果 研究组治疗有效率为77.78%,比对照组的53.33%明显升高(P<0.05);研究组白细胞减少、肝损伤的发生率均显著低于对照组(P<0.05)。对照组治疗后外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平比治疗前明显降低,而研究组均明显升高(P<0.05);两组治疗后中医证候积分和血清癌胚抗原(CEA)、糖类抗原125(CA125)、鳞状上皮细胞癌抗原(SCCA)水平较治疗前显著降低,且研究组比对照组更低(P<0.05)。研究组1年、2年生存率分别为93.33%和77.78%,均明显高于对照组(P<0.05)。结论 血府逐瘀汤联合标准TP化疗方案能进一步显著改善宫颈癌患者的临床症状、免疫功能,有效降低肿瘤标志物水平,减轻化疗毒副作用。Objective To investigate the effects of Xuefu Zhuyu Decoction combined with standard TP regimen(paclitaxel + nedaplatin) on chemotherapy toxicity and survival of patients with Qi stagnation and blood stasis cervical cancer. Methods 90 patients with cervical cancer with Qi stagnation and blood stasis syndrome who received treatment in Tianchang Hospital of Traditional Chinese Medicine of Anhui Province from December 2015 to November 2019 were selected as research objects. According to the randomized control principle, they were divided into 1∶1 groups, with 45 cases in each group. All patients were treated with standard TP regimen, and the study group was treated with Xuefu Zhuyu Decoction at the same time. The chemotherapy efficacy, toxic side effects and quality of life of the two groups were compared. Results The effective rate of the study group was 77.78%, which was significantly higher than that of the control group(53.33%)(P<0.05). The incidence of leukopenia and liver injury in the study group were significantly lower than those in the control group(P<0.05). After treatment, the levels of CD3^(+), CD4^(+)and CD4^(+)/CD8^(+)in peripheral blood of the control group were significantly lower than before treatment, while those of the study group were significantly higher(P<0.05). TCM syndrome scores and serum levels of carcinoembryonic antigen(CEA), carbohydrate antigen 125(CA125) and squamous cell carcinoma antigen(SCCA) in both groups were significantly decreased after treatment, and the levels in the study group were lower than those in the control group(P<0.05). The 1-year and 2-year survival rates of the study group were 93.33% and 77.78%, respectively, which were significantly higher than those of the control group(P<0.05). Conclusions Xuefu Zhuyu Decoction combined with standard TP chemotherapy can significantly improve the clinical symptoms and immune function of patients with cervical cancer, effectively reduce the level of tumor markers, and alleviate the toxic side effects of chemotherapy.
