摘　　要：目的 通过检测尖锐湿疣患者皮损组织中杆状病毒凋亡抑制蛋白5(BIRC5)、半胱氨酸天冬氨酸蛋白酶9(CASP9)表达变化,分析其临床意义。方法 选取2018年1月至2021年10月杭州市余杭区第二人民医院诊治的110例尖锐湿疣患者设为尖锐湿疣组,治疗后观察3个月,其中有39例复发(复发组)、71例未复发(未复发组)。对照组(n=80)为行外阴整形术的32例女性及行包皮环切术的48例男性,皆于术中留取正常组织。比较各组BIRC5、CASP9表达水平,并分析BIRC5、CASP9表达与尖锐湿疣患者临床病理参数的关系。采用受试者工作特征曲线评估BIRC5、CASP9对治疗后复发的诊断价值。结果 尖锐湿疣组皮损组织BIRC5表达水平高于对照组,CASP9表达水平低于对照组(P<0.05)。人乳头瘤病毒混合感染、疣体团体直径≥10mm、病程≥3个月的尖锐湿疣患者BIRC5表达水平较高,CASP9表达水平较低(P<0.05)。复发组BIRC5表达水平高于未复发组,CASP9表达水平低于未复发组(P<0.05)。BIRC5联合CASP9诊断复发的曲线下面积为0.966。结论 BIRC5、CASP9在尖锐湿疣患者皮损组织中异常表达,复发患者BIRC5表达水平高于未复发患者,CASP9表达水平低于未复发患者,BIRC5联合CASP9对治疗后复发有一定诊断价值,并可能参与尖锐湿疣的发生、发展过程。Objective To detect the expression changes of baculovirus inhibitor of apoptosis protein 5(BIRC5) and caspase 9(CASP9) in skin lesions of patients with condyloma acuminatum, and to analyze their clinical significance.Methods A total of 110 patients with condyloma acuminatum treated in the Second People′s Hospital of Yuhang District were selected as condyloma acuminatum group, and observed for 3 months after treatment.Among them, 39 cases recurred(recurrence group) and 71 cases did not recurred(non-recurrence group).The control group(n=80) consisted of 32 females who underwent vulvar plastic surgery and 48 males who underwent circumcision, and normal tissues were obtained during the operation.The differences in the expression levels of BIRC5 and CASP9 in each group were compared, and the relationship between the expression of BIRC5 and CASP9 and the clinicopathological parameters of patients with condyloma acuminatum was analyzed.The diagnostic value of BIRC5 and CASP9 for recurrence after treatment was evaluated by receiver operating characteristic curve.Results The expression level of BIRC5 in skin lesions of condyloma acuminatum group was significantly higher than that of control group, and the expression level of CASP9 was lower than that of control group(P<0.05);patients with human papilloma virus mixed infection, wart group diameter ≥10mm, and disease duration ≥3 months had higher expression level of BIRC5, but lower expression level of CASP9(P<0.05);the expression level of BIRC5 in the recurrence group was higher than that in the non-recurrence group, and the expression level of CASP9 was lower than that in the non-recurrence group(P<0.05).The area under the curve of BIRC5 combined with CASP9 for the diagnosis of recurrence was 0.966.Conclusions BIRC5 and CASP9 are abnormally expressed in the skin lesions of patients with condyloma acuminatum.The expression level of BIRC5 in the recurrence patients is higher than that in the non-recurrence patients, and the expression level of CASP9 is lower than that
